Seventy years ago today, nineteen firefighters ran toward a burning tank to save it — and the tank killed them. Not because they were reckless. Because nobody yet understood what a tank full of fire is actually counting down to. Here’s the story, and the doctrine that’s kept crews alive since.

On This Day in Safety — July 29, 1956 · McKee Refinery, near Sunray, Texas

It was a Sunday, a little before 6:00 in the morning, at the Shamrock Oil & Gas McKee Refinery, seven miles south of Sunray in the Texas Panhandle. On the tank farm sat a Horton sphere — one of those big steel “pumpkin” tanks — holding about 500,000 gallons of pentane and hexane. Light hydrocarbons. The kind that flash to vapor and roll downhill looking for a spark.

Vapor had gotten loose. It drifted down toward the other installations, found an ignition source — probably over near the asphalt plant — and lit. The flame front raced back up the vapor trail to the sphere and wrapped it in fire.

The call went out. Volunteer crews from Sunray and Dumas rolled in alongside Shamrock’s own plant fire brigade, and they went to work the way everybody was trained to work a tank fire in 1956: get in, cool the steel, protect the neighboring tanks, knock it down. At some point the decision was made to lower the liquid level in the burning sphere — to draw some of the product out. It seemed reasonable. It was the worst thing they could have done. Dropping the liquid level exposed more bare steel above the liquid line to the flame — and bare steel, with no liquid behind it to carry the heat away, gets hot and goes soft fast.

A few minutes before 7:00 a.m. — about an hour after the first flames — the sphere let go.

This is what a BLEVE is, and why the word still stops a firefighter cold: a Boiling Liquid Expanding Vapor Explosion. Fire heats the shell above the liquid line. That steel weakens until it can’t hold the pressure inside. When it tears open, the superheated liquid — pentane and hexane sitting well above their boiling point, held liquid only by pressure — flashes to vapor all at once, expanding hundreds of times over in an instant, and ignites. The sphere came apart. The blast lit three more storage tanks.

Sixteen firefighters died right there. Three more died later of their burns. Nineteen gone — seven from Dumas, eight from Sunray, four from Shamrock’s own brigade. Men named Ray Biles, Billy Joe Dunn, Gilford Corse, Sam Gibson, Wayne Thomas — the youngest of them just twenty-five and twenty-seven years old. Thirty-two more people were hurt, firefighters and onlookers both. The fire was finally just left to burn itself out overnight. By firefighter deaths in a single incident, it remains one of the worst days in the history of the American fire service.

Here’s the floor translation, and it comes in two pieces.

For the people who store the stuff: pentane and hexane are Class I flammable liquids, and 29 CFR 1910.106 governs how you keep them. It’s not decoration. It requires emergency relief venting sized for fire exposure — so a tank cooking in a fire vents its pressure instead of tearing itself apart — plus tank spacing and drainage so one tank on fire doesn’t cook the four next to it. Every one of those requirements is an answer to a day like this one.

For the people who show up when it’s already burning: a pressure vessel wrapped in flame is not a fire to fight. It’s a clock. The fire service rewrote its whole approach after fires like McKee — you pull back to the evacuation distance, you cool from a distance with fixed or unmanned monitors if you cool at all, and you do not de-inventory a fire-exposed tank by dropping its liquid level, because that’s exactly what feeds the failure. Nineteen men learned that for the rest of us. The lesson isn’t “be braver.” It’s that a tank in fire will BLEVE with no warning — so the win is measured in how far away your people are.

The Full EHS Picture

Most days the story is bigger than the “S,” so let’s be straight about all three. (S) The safety toll was brutal and immediate: nineteen firefighters dead, thirty-two injured, at the tank. (H) The health footprint here was acute, not chronic — catastrophic thermal burns, severe enough that three men who survived the blast died of them in the days after; this was fire and pressure, not a slow poison. (E) On the environment, I won’t dress it up: this was a combustion-and-blast event in a rural refinery, and there’s no documented record of a persistent soil or water contamination legacy — roughly half a million gallons of pentane and hexane, plus three more tanks, burned off into a towering hydrocarbon smoke plume over the Panhandle rather than leaching into a creek. The offsite reach here was the blast and the smoke, not a Superfund site. But make no mistake about the modern frame: a tank farm like that today lives under EPA’s Clean Air Act rules and, above threshold quantities of a flammable substance, EPA’s Risk Management Program (RMP) — and any firewater running off mixed with product would be a Clean Water Act/NPDES problem. One under-protected vessel in a fire is a safety event, a health event, and an environmental event at the same time. It just depends which fence line you’re standing at.

A tanker trailer killed a worker in Corpus Christi — and OSHA is calling it a confined space. On July 15, OSHA cited big-rig parts distributor FleetPride Inc. for 16 serious violations after an employee asphyxiated while inspecting a tanker trailer back in January. The findings: no confined space program, a respiratory protection program missing key pieces, and electrical hazards on top of it. Proposed penalty, $264,380. So what for safety leaders: a tank, a trailer, a vessel you climb into “just to look” is a permit-required confined space under 29 CFR 1910.146, and the killer is usually not something you can smell — it’s an atmosphere that’s simply run out of oxygen or filled with something that displaced it. If your people ever enter tanks, totes, trailers, or pits, the permit, the air test before entry, and a rescue plan that doesn’t send a second body in after the first are the whole ballgame. Same lesson as the sphere: the vessel that looks routine is the one that gets you.

Seven inspections, seven times the same violation. On July 23, OSHA cited Florida roofer Orchids Builders LLC for two willful and four repeat violations — $349,754 proposed — after inspectors twice found crews exposed to 9- and 10-foot fall hazards with no fall protection, plus missing training records, no eye protection on nail guns, and ladders that didn’t extend the required 36 inches above the roofline. The detail that matters: the company has been inspected seven times since 2023, and every single one found a fall-protection problem. So what: falls are still the number-one killer in construction, and 29 CFR 1926.501 is the most-cited standard in the country for a reason. “Willful” and “repeat” aren’t bad luck — they’re a pattern a safety program is supposed to break. If you’re a sub or a GC, a crew’s inspection history is a hazard signal you can check before they ever set foot on your site.

Fail of the Day

Shared blamelessly, so the next crew doesn’t learn it the hard way.

A plant emergency response team got called to a small fire licking at the base of a propane bullet tank out by a loading rack. A newer responder did exactly what instinct and years of “protect the asset” training told him to do: grabbed a line and started moving in to cool the tank up close. A veteran on the crew caught his shoulder and pulled everyone back — past the rack, behind a concrete barrier — and they set an unmanned monitor to throw water on the shell from a distance. A couple minutes later the relief valve lifted and screamed, venting propane in a roaring plume exactly the way it’s designed to. The tank held. Nobody was near it.

Here’s the blameless read, because it matters. Nobody on that crew was a cowboy. The urge to close in and save the tank is trained into firefighters and plant crews — it’s muscle memory, not a character flaw. The system worked because one person carried the McKee lesson in his gut and had the standing to override the instinct in the moment. The fix isn’t “want it more.” It’s building the doctrine in before the alarm: fixed and unmanned monitors, a marked evacuation distance everyone knows cold, and a culture where pulling people back from a fire-exposed pressure vessel is the heroic move — not advancing on it.
