Eighty-two years ago this week, 320 men were killed in a few seconds loading ammunition onto a ship — most of them never trained for the work they were ordered to do. When the survivors were sent back to the same job under the same conditions and refused, the Navy didn’t fix the loading. It charged fifty of them with mutiny. This one’s about what happens when leaders answer a disaster by blaming the people closest to it.
On This Day in Safety — July 17, 1944 · Port Chicago, California
Straight with you: no verified major U.S. worker-safety incident lands on the exact date of July 16, so we’re stepping one day into this same week for a lesson that still lands on the floor.
A little after 10:18 on the night of July 17, 1944, two cargo ships sat at the munitions pier of the Port Chicago Naval Magazine on Suisun Bay, northeast of San Francisco. The SS E. A. Bryan was being loaded with a variety of munitions — antiaircraft ammunition, aerial bombs, high explosives, smokeless powder. In her holds and on the pier ready to go were about 4,606 tons of munitions, with another 429 tons sitting in sixteen railcars on the pier. Witnesses heard a dull clang and the sound of splintering wood, then a blinding flash. The pier, the E. A. Bryan, and everything within a thousand feet disintegrated. The second ship was blown to pieces. A Coast Guard fire barge was thrown clear and sank, taking its five-man crew with it.
Three hundred and twenty men were killed instantly — Navy loading crews, a Marine sentry, the ships’ Armed Guard and merchant crews, civilian workers. Only fifty-one could be identified afterward. Nearly two-thirds of the dead were African American sailors. More than 250 other people were injured.
Here’s the part that matters for us. The Navy’s own court of inquiry could never pin down the exact spark — too little was left. But it found the conditions plainly: the base was running at maximum wartime capacity, munitions-handling training for officers and men was uneven at best, and ever-increasing loading quotas had led the command to override basic safety procedures. The men doing the most dangerous work on the pier were segregated Black enlisted sailors, most with no ordnance training, working under white officers who often had little training or leadership experience themselves. The people closest to the explosives had been handed the job without being handed the knowledge to survive it.
The rule, translated to the floor: Port Chicago predates OSHA by a quarter century, so I won’t pretend it wrote a CFR — that would be a lie, and we don’t do those here. But the work those men did — handling and staging mass quantities of explosives — is governed on today’s civilian floor by 29 CFR 1910.109, explosives and blasting agents, and its process-safety cousin 1910.119. Read them out on the floor and they say four simple things: limit how much energetic material is staged and exposed at one time, keep quantities separated by distance, let only trained people handle it, and run the whole thing off written procedures — with a crew that can stop the job. Port Chicago broke every one of those in spirit: thousands of tons, untrained crews, the schedule beating safety every shift.
And if you think that’s only history: on October 10, 2025, at 7:47 in the morning, the Accurate Energetic Systems explosives plant in McEwen, Tennessee blew up. Sixteen workers who were hand-pouring cast boosters were killed and seven more were hurt. The CSB found roughly 24,600 pounds of explosive material in that one building; about 23,000 pounds of it went off. The blast was felt twenty miles away and registered as a magnitude-1.6 earthquake. Eighty-one years after Port Chicago, the same lesson wrote itself again: the more you expose in one place, the more people a single event can take.
Now the hardest part of the story. Three weeks after the blast, the surviving loaders were transferred to Mare Island and put right back on ordnance duty — same work, same conditions. On August 9, 1944, hundreds of them refused to load until it was made safe. Under threat of a mutiny charge, most went back. Fifty men held their ground. The Port Chicago 50 were convicted of mutiny on October 24, 1944, and sentenced to eight to fifteen years; more than two hundred others took summary courts-martial. A young NAACP lawyer named Thurgood Marshall carried their fight. The men were released in 1946, but the convictions stood for decades. On July 17, 2024 — the eightieth anniversary — the Navy exonerated the Port Chicago 50 and more than two hundred others, 256 sailors in all, after its General Counsel found significant legal errors in the trials.
That’s the HOP read, handed to us by history. Nobody at Port Chicago chose to die. The explosion was the system — max tempo, no training, quotas over safety. And when the men closest to the work saw the danger and refused, leadership called it mutiny instead of fixing the loading. How leaders answer failure is the whole story: in 1944 they court-martialed the crew; in 2024 they admitted the crew had been right all along. The person closest to the work saw the hazard first. They almost always do.

Hydrogen sulfide killed two young engineers in Maine — and the CSB just laid out how (July 14). The Chemical Safety Board released an update on the January 27, 2026 hydrogen sulfide release at the Woodland Pulp mill in Baileyville, Maine. During a mill shutdown, automatically-dosed sulfuric acid reacted with high-pH, sulfur-bearing fluids in an acid sewer line and generated H2S; when the scrubber fan was switched off as part of the shutdown, the gas had nowhere to go but into the building. A 20-year-old co-op engineering student and a 26-year-old engineer — both new to the company — died. Ten others were exposed. The CSB found no personal H2S monitors on the workers, no fixed H2S detectors in that building, no ventilation, and no system tracking who was inside — the two men weren’t found for hours. So what for safety leaders: your normal controls can switch off with the plant. A shutdown or turnaround changes the atmosphere, and the protections you count on during normal ops — that scrubber, that fan — may be exactly what gets turned off. Personal gas monitors on people, fixed detection in the space, ventilation, and knowing precisely who is in the building during an upset aren’t extras. They’re the gap between an exposure and two funerals.
$343K for a trench in Texas (July 15). OSHA fined a Houston utility contractor $343,000 after a worker was hospitalized in an excavation collapse at a Brazoria County work site. So what: cave-ins are 100% preventable and still burying people. A single cubic yard of soil weighs about 3,000 pounds — a small car sitting on your chest. Once you’re five feet down, 29 CFR 1926 Subpart P is not a suggestion: slope it, shore it, or shield it; keep the spoil pile back from the edge; put a ladder within twenty-five feet of every worker; and have a competent person inspect it before anybody drops in. If you can’t name which of the three protections you’re using, nobody goes in the hole.
Fail of the Day
A turnaround crew is cutting out old pipe up on a rack. The hot-work permit was pulled that morning, fire watch posted, everything by the book. As the shift wears on, the laydown crew keeps staging drums of solvent and totes of flammables closer and closer to the base of the rack — it’s the handy spot, and the pile grows a little at a time. Nobody ever decided to stack flammables under the cutting. It just crept. Mid-afternoon a pipefitter glances down, sees sparks landing a few feet from the drums, and calls the job. The supervisor backs him without a word. They move the staging, re-check the permit, and go back to work. No fire.
Read it blameless, because that’s where the lesson is. Nobody broke a rule. The permit cleared the cutting; it never capped how much flammable could sit at the point of work, and the staging drifted as the job evolved. The save wasn’t the paperwork — it was a worker who could stop the job and a boss who didn’t argue. That’s the Port Chicago lesson in miniature: the person closest to the work saw it first, and this time the system let him act. Don’t ask who stacked the drums. Ask why nothing capped the quantity — then cap it.

Do This One Thing
Count what’s exposed. Before work starts, walk the point of work and look at how much hazardous material is actually staged there right now — flammables, energetics, gas, product — not what the permit allows in theory, but what’s physically sitting there. Cut it to the minimum the job needs and put distance between your people and the rest. Port Chicago lost 320 and McEwen lost 16 for the same underlying reason: too much in one place with people on top of it. Then make sure anyone on that crew can stop the job without a fight. The people closest to the work see the hazard first — build the job so they can act on it.
