Twenty-seven years ago this week, three ironworkers climbed into a steel basket and let a crane hoist them into the sky to watch the biggest lift of the whole job go into place. Below them hung 400 tons of stadium roof on the hook of one of the largest cranes on earth. The crane was rated for that load. Nobody had done the math on the load plus the wind. (There’s no verified major workplace-safety incident on the exact date of July 13, so today we mark one from this same week — July 14, 1999.)

On This Day in Safety — July 14, 1999 · Milwaukee, Wisconsin

A little after 5 p.m. on July 14, 1999, the crew at the Miller Park stadium site in Milwaukee was setting a piece of the retractable roof. The crane doing the work was a Lampson LTL-1500 Transi-Lift — a crawler crane so big it had its own nickname, “Big Blue,” and at the time it was one of the largest cranes in the world. On the hook: a roof section weighing better than 400 tons.

Three ironworkers from Iron Workers Local 8 — Jeffrey Wischer, William DeGrave, and Jerome Starr — were riding in a suspended personnel platform, hoisted up alongside the pick to observe and guide it into place. The wind was up that evening, gusting hard; the engineering analysis afterward put it around 23 mph sustained with gusts near 35. At 5:12 p.m. Big Blue came down. The falling crane struck the man-basket. All three ironworkers were killed. Five other workers were hurt.

Here’s the part that turns it from a bad-luck story into a lesson. OSHA’s investigation found the crane was being operated outside its design limits for the combination of load and wind. The roof panel on the hook wasn’t just dead weight — hung up there in a gusting crosswind, a 400-ton slab becomes a sail. The company that designed and installed the roof had calculated the side-wind forces on the crane itself, but did not account for the wind load pushing on the suspended panel. Add that force back in and the pick was over what the machine could safely hold. Investigators also flagged a king pin that hadn’t been properly installed or maintained, and a plain lack of coordination among the contractors on the lift. Three firms — Mitsubishi Heavy Industries America, Lampson International, and Danny’s Construction — were later cited, with penalties totaling more than half a million dollars (OSHA news release, January 12, 2000).

Now the rule, translated to the floor. In 1999, cranes in construction lived under the old Subpart N — 29 CFR 1926.550(a)(1) — which said one simple thing: follow the manufacturer’s specs and limits, and don’t exceed the rated capacity. Real, but thin. The pattern of crane-collapse tragedies through the 1980s and 1990s — Big Blue among the loudest of them — is what finally drove a decade-long push to a real standard. OSHA published Cranes and Derricks in Construction — 29 CFR 1926 Subpart CC — on August 9, 2010, effective that November. Two pieces of it read like they were written at this exact lift:

1926.1417, operation: You run the crane inside the manufacturer’s load chart for the conditions you’re actually in. Wind on the load is part of the load. You don’t get to pretend the air is still because the schedule wishes it were.

1926.1431, hoisting personnel: You do not hoist people on a crane when the wind, sustained or gust, tops 20 mph unless a qualified person looks at it and says it’s safe. Wischer, DeGrave, and Starr were riding a basket in gusts nearly double that number.

Read it through the window of that basket and it stops being paperwork. The men were not reckless. They did a normal thing — ride up to watch and steer the most important pick of the job, the way ironworkers had done on big lifts for years. What killed them was a system that loaded a crane past what it could hold in the wind that was actually blowing, and then put three humans in the sky in that same wind with no margin. The wind wasn’t background noise. It had a vote in whether that pick was safe. Nobody counted it.

The rule is the rulebook written in their names. Count the wind — on the load and on the people — before it counts you. A bronze memorial called Teamwork stands at the ballpark today for the three of them. The rule is the other memorial.

Hurricane season is live, and the CSB says don’t wait for the forecast to get religion. On June 2, the U.S. Chemical Safety Board urged chemical and process facilities to prepare now for the Atlantic hurricane season, which runs June 1 to November 30. Even with NOAA predicting a below-normal season, the Board’s point is blunt: it only takes one storm hitting one unprepared plant. They point straight at two of their own investigations — Arkema in Crosby, Texas, where Hurricane Harvey in 2017 knocked out refrigeration and backup power and let organic peroxides cook off into a fire, and Bio-Lab in Westlake, Louisiana, where Hurricane Laura in 2020 tore roofs off buildings, let rainwater reach a million-plus pounds of reactive pool chemical, and pushed a chlorine plume over Interstate 10. So what for you: if a storm can reach you, the four things that save you are boring and they work — secure your hazardous materials, guarantee backup power to the safety-critical systems, drill your people on the emergency plan, and coordinate with local responders before the cone shows up on the map. The plan you write in July is the one that holds in September.

A tank crushed by nothing, and it’s the CSB’s newest deployment. The Board’s most recent field investigation isn’t an explosion; it’s the opposite. In the early morning of May 26, a large tank holding “white liquor” — a caustic solution used in pulp and paper — ruptured and imploded at the Nippon Dynawave packaging mill in Longview, Washington, killing workers and injuring others. So what for you: implosion is the hazard people forget. A storage tank is built to hold pressure in; it has almost no strength against a vacuum. Steam-clean it and let it cool with the vents shut, drain it fast with the vent valve plugged, or pull suction on it the wrong way, and the atmosphere outside will fold it like a beer can — with people on or around it. If you run big atmospheric tanks, the question this week is simple: what keeps a vacuum from forming when we heat, cool, drain, or clean this thing — and is that vent guaranteed open?

Fail of the Day

A small crew is setting a steel beam with a rough-terrain crane late in the day, wind starting to move the trees at the edge of the lot. It’s the last pick before they can go home. The load comes off the ground and immediately wants to weathervane — swinging, hunting, tugging the tag line hard enough to walk the guy holding it a couple steps. For a second the beam drifts toward two workers waiting to guide it in. The operator sets it back down on his own call before anyone gets between the load and the building. Nobody hurt. Everybody quiet for a minute afterward.

Read it blameless, because that’s where the lesson is. Nobody was a cowboy. They did the most human thing on a jobsite — one more pick so we’re not back out here tomorrow. There was no posted wind limit for that crane and that load, no anemometer anybody was actually watching, and no agreed trigger for who calls it off and when. So the decision defaulted to the operator’s gut in the moment — and this time his gut was fast enough. The fix isn’t “be more careful in the wind.” It’s a number on paper (this load, this crane, stop at this wind), a way to see the wind, and one named person with the standing to kill the pick with no argument. Give the crew a line they don’t have to be a hero to draw, and the beam never gets a run at anybody.

Before your next crane pick, settle three things out loud with the crew: the wind number, the way you’ll see it, and the person who owns the stop. What wind speed shuts this specific lift down — off the load chart, not off a hunch? How are we actually measuring it — a real anemometer someone’s watching, not a wet finger? And who has the authority to say “set it down” without needing a vote? While you’re at it: nobody rides the load or the hook, and nobody stands under it. Wischer, DeGrave, and Starr were up in that basket because watching the pick from the sky felt like the careful thing to do. The rule they bought us says keep people out of the air and out from under the steel — and count the wind before it counts you.
