Ask a room full of folks what lockout/tagout is for and most will say “so the machine doesn’t start while my hand’s in it.” True. But LOTO isn’t just about breakers and disconnects. A pipe full of flammable gas is stored energy too — and a valve you believe is closed is a promise, not a proof. Sixteen years ago today, a coke plant on the Monongahela learned that the hard way.
On This Day in Safety — July 14, 2010 · Clairton, Pennsylvania
At U.S. Steel’s Clairton Works — the largest coke operation in the country — a crew was sent to fix a leaking valve on a 30-inch coke oven gas line over in the battery area. Coke oven gas is nasty stuff: it’s loaded with hydrogen, methane, and carbon monoxide — flammable and toxic in the same breath. The plan was to slide a steel blank between the flanges to block the gas while they worked the valve.
The line wasn’t dead. The gas found an ignition source, and it went up. Workers from both U.S. Steel and the contractor, Power Piping, were caught in it — men walked away with first-, second-, and third-degree burns, and more than a dozen were hurt. That anyone lived through it was luck, not design.
When OSHA finished, the finding was blunt. U.S. Steel drew two willful violations and 11 serious; Power Piping drew six serious; $175,000 in proposed penalties. The willful charge wasn’t fancy — it was the failure to provide an effective energy control procedure. “U.S. Steel and Power Piping did not have the proper controls in place to prevent worker exposure to hazardous energy,” said the director of OSHA’s Pittsburgh office.
The rule it points to: 29 CFR 1910.147 — The Control of Hazardous Energy (Lockout/Tagout). Here’s the floor translation. Energy isn’t just electricity. It’s the gas in the line, the steam in the header, the product sitting behind a valve that’s “closed” but quietly passing. Before you break into that line, you isolate it, you blind or double-block-and-bleed it, you drain and purge it, and then you verify zero right where you’re going to open it. A closed valve can leak by and you’ll never know until the flange is loose and the gas is on your face. Clairton is why the verify step exists — it was written in burns.

A crawl space is still a confined space (San Antonio, TX — July 13). OSHA cited a building contractor and the staffing company that supplied the worker after a man was fatally injured running a mini-excavator in the crawl space beneath an elementary school in Converse. Two things for leaders: a low, tight space under an occupied building stacks confined-space hazards on top of struck-by and caught-between hazards from the machine — that needs a real plan, a spotter, and a way out. And note who got cited: the host contractor and the staffing agency both. If you use temp or leased labor, you share the duty to train and protect them. It isn’t “their employee, their problem.”
The emergency isn’t over when the leak stops (Channelview, TX — late June). OSHA proposed more than $3.5 million against three employers for failing to protect workers during the post-spill cleanup at the BWC Terminals facility. The lesson safety leaders keep relearning: the cleanup phase is still hazardous work. Air monitoring, the right respiratory and skin protection, and a written plan don’t get to clock out just because the visible crisis did.
Fail of the Day
A maintenance tech goes to pull a pump for a seal change. He closes the block valve upstream, watches the gauge settle, and cracks the flange. He gets a face-full of hot product — the valve was passing, seat worn just enough to let it by. He’s fine: he was standing to the side, face shield down, and it caught him on the shoulder instead of the eyes.
Easy to say “he should’ve checked.” But look at the system, not the man. The procedure told him to close the valve — it didn’t tell him to prove the line was empty with a bleeder before he opened it. He did exactly what he was trained to do. The gap was in the plan, not the person. Add the verify-zero step and this near-miss never happens. That’s the whole point of Just Culture: fix the trap, not the guy who stepped in it.

Before anyone breaks into a line today — gas, steam, or product — make them prove it’s dead. Isolate, blind or double-block-and-bleed, drain, and crack a bleeder or verify with a monitor at the point of work. A closed valve is a promise. A verified zero is proof. That’s 1910.147, and that’s the difference between a seal change and a burn ward.
