Good morning. Today’s story is 130 years old, and it still happens — in trenches, in vaults, anywhere the ground is quietly holding its breath over somebody’s head. The men in it heard the danger coming for two weeks. That’s the part that should keep us up at night.

On This Day in Safety — June 28, 1896 · Pittston, Pennsylvania

One hundred thirty years ago this morning, ninety men and boys were cutting coal more than four hundred feet under Pittston, Pennsylvania. For two weeks the rock above them had been groaning — a sound the old hands called the squeeze. At three in the morning on a Sunday, two hundred acres of roof came down at once, and fifty-eight of them never came back up.

It was the Newton Coal Company’s Twin Shaft Colliery, in the Red Ash vein. The crew was “robbing the pillars” — going back into the worked-out sections and cutting away the very columns of coal that were holding up the mountain, to squeeze the last tonnage out of the seam. Take enough of the pillar and there’s nothing left to carry the weight overhead. Everyone knew it. That’s why the ground was talking.

And it had been talking for two weeks. The surveyors had told the general superintendent the mine was “squeezing.” Props went in. The crackling got worse, not better. One miner — Edward Hughes — testified later that he defied his boss and walked off his shift that night “because the crackling grew worse.” He lived. The men who stayed were sent in to prop a roof that was already failing.

At 3 a.m. the concussion was felt for miles. Windows rattled across Pittston; people were thrown from their beds thinking it was an earthquake. This was no fire or gas blast — it was the air being driven out as two hundred acres of overburden dropped at once. Fifty-eight men and boys were buried 434 feet down. Among them: M.J. Lanigan, 49, the inside superintendent and the town’s acting mayor; M.J. Lynott, 43, the inside foreman; Thomas Tenpenny, 24, the fire boss; and Frank Kehoe, just 16. They left 31 widows and 101 orphans. Not one body was ever recovered. The mine is their grave.

Pennsylvania’s governor ordered a formal inquiry. The commission’s 1897 report (Stein, Roderick and Brennan) named the cause plainly: chambers cut too wide, pillars robbed of their coal, nothing left to hold the roof. Its core recommendation reads like the first draft of every ground-control rule we have — leave the pillars standing, don’t rob them, and give the inspectors maps of the workings.

That’s the bloodline of modern ground control. In the mines today it’s the roof-control plan — 30 CFR 75.220. On your job site it’s 29 CFR 1926 Subpart P — Excavations: slope it, shore it, or shield it (1926.652), and a competent person who inspects the ground every shift and pulls people out at the first sign of movement (1926.651(k)). A trench is just a mine roof stood on its end. A single cubic yard of soil weighs up to about 3,000 pounds — the weight of a small car — and when it comes in it doesn’t knock you down, it sets like wet concrete around your chest and you suffocate standing up. “Robbing the pillars” has modern names too: pulling the box to reach one more fitting, sloping nothing because the spoil’s in the way, dropping in “just for a second.”

These men were not reckless. They were the most experienced hands in the valley, doing exactly what the operation was built to do — get the coal — and when the ground started talking, the plan was to send men under it to shore it up instead of clearing out and staying out. Edward Hughes, the man closest to the work, heard the hazard first and trusted his ears. The system never gave the other fifty-eight that choice. The rule is the rulebook written in those people’s names. Build the barriers so the instinct — just prop it, just one more cut, just a minute — never gets the chance.

The ground is still winning. Two weeks ago, June 15–19, the country marked National Trench Safety Stand Down Week, run by the National Utility Contractors Association. The timing lands hard against today’s story. Cal/OSHA used the week to report it is investigating two Los Angeles-County trench-collapse deaths already this year — on top of roughly 90 trench-safety violations it wrote from 2021 through 2025. Recent California cases show how routine the gamble still is: a San Diego public-utilities job drew a $297,850 proposed penalty after a trench caved on two workers, and a South San Francisco sewer job ($529,640 in combined fines) was referred for felony charges. So what: the agency line hasn’t changed since 1896 — slope it, shore it, or shield it, regardless of depth. The stand-down materials are free. The cheapest control you own this summer is a five-minute toolbox talk and a competent person who is allowed to say “everybody out.”

Heat is the hazard with no trench box. OSHA’s updated Heat National Emphasis Program took effect this spring (April 10, 2026) and is active now, with a standardized framework (Appendix J) that makes it easier for inspectors to cite the missing basics: water, rest, shade. The federal heat standard is still stuck in proposed rulemaking — so the NEP, riding on the General Duty Clause, is how heat actually gets cited today. So what: the workers who go down are almost always new, or just back from time off — bodies that haven’t acclimatized. OSHA’s own rule of thumb is the Rule of 20%: a new worker does no more than 20% of a full heat workload on day one and builds up across the week. Got new crew this summer? That’s your highest-risk person on the hottest afternoon.

Fail of the Day

Six-foot trench, residential sewer tie-in. The trench box was on the truck — but it wouldn’t fit down around the existing fittings, and the homeowner was standing right there watching. The plan was “in and out in two minutes” to glue one coupling. The spoil pile was right at the lip, because the yard was tight. One guy hopped in. He felt the wall tick against his boot — a little slough of dirt, nothing dramatic — and his partner grabbed his wrists and hauled him out on reflex. Ten seconds later a slab of sidewall the size of a door let go right where his knees had been.

Nobody got hurt. Nobody was stupid. The box genuinely didn’t fit the work, the clock was genuinely running, and “two minutes” is the most reasonable-sounding sentence on any job site. That’s exactly what makes it dangerous — the trap wasn’t a careless person, it was a setup where the fast way and the safe way pointed in opposite directions and somebody had to choose under a customer’s eyes. The fix isn’t “be more careful.” It’s making the safe way the easy way: the right protection staged before the dig, spoil set back two feet, and a crew that treats “I felt it move” as a full stop, not a maybe.

Before anybody’s body goes below grade today — trench, pit, vault, doesn’t matter — point at the protection and say it out loud: sloped, shored, or shielded? If the honest answer is “none of the three,” that’s not a trench, it’s an open grave, and the work waits. Then put a name on the wall: one competent person whose only job at the edge is to watch the ground and call everyone out the second it starts to talk. Twin Shaft heard it coming for two weeks. You might get two seconds. Build the system so two seconds is enough.
