Nineteen years ago today, three men went into a Utah coal mine to bring six others home. That mountain had been cracking and shifting for ten straight days — everyone underground could feel it. They went in anyway. It took them too.
On This Day in Safety — August 16, 2007 · Crandall Canyon Mine, Emery County, Utah
In the early-morning dark of August 6, 2007, six miners were running retreat mining more than a half-mile back and nearly 1,800 feet under the top of the Wasatch Plateau. Retreat mining is exactly what it sounds like: you’ve taken the coal you easily can, so now you pull out the pillars you left behind to hold the roof up, and you back out while the mountain caves in behind you. It’s legal. It’s also the last, most dangerous thing a mine does before it’s abandoned — because the entire plan depends on the ground failing where you want it to, when you want it to.
It didn’t. The pillars let go all at once in a violent coal “bump” — an outburst that threw coal across a half-mile of tunnel and registered on seismographs as a magnitude-3.9 event. Kerry Allred, Luis Hernandez, Carlos Payan, Don Erickson, Manuel Sanchez, and Brandon Phillips were trapped behind it. They were never heard from again.
For ten days, crews drilled and dug toward them. And for ten days, the mountain kept bumping — the same overstressed pillars kept bursting as rescuers tunneled through the same failing ground. On August 16, it burst straight into the rescue tunnel. Dale “Bird” Black, 48, and Brandon Kimber, 29 — both miners — were killed. So was Gary Jensen, 53, an MSHA inspector who had gone underground to help reach the trapped men. Six others were hurt. On September 1 the search was called off. That November, the operator sealed the passageways with concrete. The first six men are still in there.
MSHA’s investigation was blunt about why. The mine’s design and its pillar-recovery plan were inadequate for that depth, and after earlier coal bursts warned the crew the ground was overstressed, the plan wasn’t changed. The operator had pulled coal the federally approved roof-control plan required them to leave standing. That plan — the roof-control plan every underground mine must follow and revise when conditions change — lives at 30 CFR 75.220. It exists because roof, rib, and pillar failures kill miners faster than almost anything else underground, and because “the coal’s still good, keep pulling pillars” is a call that gets made above the working face, by people who never feel the bumps. MSHA proposed $1.85 million in penalties; the operator later settled for about $1.1 million including “flagrant” violations, and the outside engineering firm that blessed the pillar plan paid $100,000. A federal review also faulted MSHA for approving the plan in the first place.
Here’s the floor translation, and it isn’t only for miners. That mountain was talking. Twenty-eight seismic events big enough to detect had already rattled the area in 2007 before the roof ever came down. The men closest to the work felt every bump. The decision to keep driving a tunnel through ground that was actively failing wasn’t made by the rock — it was made under the unbearable weight of six trapped men and a ticking clock. That pressure is real, and it is human. But rock does not care how badly you need to reach someone. A rescue plan that ignores why the first collapse happened isn’t a rescue plan. It’s the second collapse, scheduled.
The Full EHS Picture
(E) Environmental — this one stayed underground. There’s no verified offsite release: no creek poisoned, no cloud over a town. The coal that burst stayed in the mountain, and the workings were later sealed and the mine idled. With deep coal the lasting environmental thread usually isn’t an acute spill — it’s what’s left behind: sealed voids, methane, and a reclamation obligation on the land above, administered under the Surface Mining Control and Reclamation Act.
(H) Health — beyond the nine dead, six rescuers were injured in the August 16 burst, and six families never got a body back to bury, a specific and permanent kind of harm no injury log captures. Underneath it all runs the slow health toll of the trade: NIOSH is still documenting a resurgence of black lung and silicosis in coal country, including in younger miners.
The tie — most disasters are E, H, and S at once. Here the S is loud and the E is quiet, but the through-line is a single pressure: chase the last tons of coal past the point the ground, the lungs, and the land can carry, and something always gives.

Decommissioning is not “the easy part.” On August 13, the U.S. Chemical Safety Board issued an update on the April 22, 2026 hydrogen-sulfide release at the Ames Goldsmith / Catalyst Refiners silver-catalyst plant in Institute, West Virginia — two workers killed, about 30 injured. Investigators found the H2S was generated during a chemical transfer into a wastewater receiving tank as the plant was being decommissioned, and that workers weren’t required to wear respirators and had no personal gas monitors. So what for safety leaders: idle and decommissioning work feels low-risk because “the process is shut down,” which is exactly when lines get opened, incompatible chemistries get mixed, and nobody is watching the atmosphere. H2S can drop a person in a breath or two. If you’re breaking into a system, someone wears the monitor and someone wears the air — 1910.134 doesn’t take a holiday because production stopped.
The heat rule is stalled; heat enforcement is not. OSHA’s federal heat standard is still stuck in rulemaking, but the revised 2026 Heat National Emphasis Program is very much active — and it makes a lack of acclimatization a primary trigger for a citation. Practical read: an inspector who shows up for a fall-protection or trench call is now told to look for heat hazards while they’re on site, and “new guy, first hot week, no ramp-up” is the pattern they’re hunting. You don’t need a final rule to be cited under the General Duty Clause. The cheapest fix in the book is an acclimatization schedule — new hires eased in over about a week, and anyone back from time off ramped up, not thrown into the deep end in mid-August.
Fail of the Day
A maintenance crew got sent back into a below-grade vault to finish a “ten-minute” repair they’d started before lunch. It had tested clean that morning, so nobody re-tested — the meter was still clipped to a bag in the truck. First guy down felt fine. Second guy, halfway down the ladder, heard the monitor the first one had finally clipped on start to scream: the vault had been quietly filling with gas the whole time the pumps ran through lunch. They both got out. Nobody got hurt.
No hero, no idiot. The trap wasn’t a careless worker — it was a completely reasonable assumption (“it was clean two hours ago”) colliding with a space that changes when you’re not looking. The fix isn’t “be more careful.” It’s a rule that doesn’t lean on memory or mood: the atmosphere gets re-tested every single time a body goes in, no matter how short the job or how recently it read clear. The monitor is cheap. The re-test is free. The assumption is what costs people.
