Most weeks in this space, somebody doesn’t come home. This week I want to tell you about the one where everybody did — nine men, 240 feet down, in cold black water for three days, and every one of them walked out. But don’t mistake a happy ending for a safe job: what nearly drowned them was a piece of paper that had been wrong since before most of them were born.

On This Day in Safety — July 24, 2002 · Quecreek, Pennsylvania

The one big July 23 anniversary — the 1984 Romeoville refinery disaster — we already ran earlier this week, so today we’re pulling from the same week. And this one earns your five minutes, because it’s the rare one that ends with everybody alive.

Around 9 p.m. on Wednesday, July 24, 2002, at the Quecreek #1 Mine in Somerset County, Pennsylvania, a continuous mining machine cut through the last few feet of coal into the wall of the abandoned Saxman Mine next door. Saxman was flooded — holding an estimated 150 million gallons of water. When the wall let go, that water came through the breach in a black wave.

Eighteen men were underground in two crews. Nine of them, on the far side, got cut off and were driven to the highest ground they could find — a space about four feet high, 240 feet below the surface. The other nine felt the air change and the water rise, and they got out and got loud. That’s what started the clock on the rescue.

For 77 hours, MSHA, the Commonwealth of Pennsylvania, and local crews drilled from the surface in the dark and the rain. Down in the hole, the trapped men huddled together against water that ran cold, shared what little they had, and — by their own account afterward — tied themselves together with cable so that if the worst happened, they’d be found as a group, and wrote notes to their families. When the first six-inch borehole finally punched through, the men banged on the drill steel: nine taps, one for each man, telling the surface all nine were still alive. Then came the wide rescue shaft and a steel capsule, and one by one, they came up. Foreman Randy Fogle went first, with chest pains. By the early hours of July 28, every one of the nine was on the surface. Not one was lost.

Here’s what put them down there. Investigators from the Department of Labor found the crew had been mining off an outdated, inaccurate map — copies dating to the 1950s — that showed the flooded Saxman workings ending well short of where they actually sat. The men believed they had hundreds of feet of solid coal between them and that old flooded mine. They had almost none. The wall they cut was the only thing holding back 150 million gallons, and the paper that was supposed to warn them had been wrong for half a century.

Name it and cite it

30 CFR 75.1200 — Mine map. Every underground coal mine has to keep an accurate, up-to-date map showing what’s been mined out, worked, or abandoned, and where water is dammed up in old workings. That map isn’t paperwork. It’s the picture of what’s on the other side of every wall you’re about to cut.

30 CFR 75.388 — Boreholes in advance of mining. When you drive toward old workings, you drill ahead — probe holes out in front of the face — so you find the water with a small hole instead of with your whole crew. The standard even spells out the distances: you drill when you come within 50 feet of workings shown by a certified survey, within 200 feet of anything not certified by survey, and within 200 feet of an adjacent mine in the same seam.

Neither of those was a new idea in 2002. They were written across a century of miners who drowned breaking into flooded old works. Quecreek is the one where that rule almost got written again — and, by luck and 77 hours of hard drilling, wasn’t. What it did do was throw a national spotlight on how many old mine maps were missing, wrong, or sitting uncollected in a drawer somewhere, and push regulators to actually go find and digitize them.

Translate it to the floor

This part isn’t just about mining. Every one of us works off a drawing somebody else made, sometimes decades ago — the as-built, the utility locate, the P&ID, the old site plan. Quecreek is what happens when you treat that drawing as the truth instead of as one person’s best guess from a long time ago. The map said solid coal. Behind the wall was 150 million gallons. On your job it’s a live line behind the drywall, a charged pipe under the slab, a tank the drawing swears is empty. So the rule on the floor is simple: the drawing is where you start, not permission to cut. When you’re getting close to something that can hurt you, you verify — drill ahead, scan, locate — because the person who cuts that last inch is betting his life on paper. And notice who saved these nine: the other crew, the guys closest to the work, who felt it go wrong first and got out and sounded the alarm. The person closest to the work sees it first. Every time.

The Full EHS Picture

(E) Environmental — the thing that nearly drowned them was abandoned-mine water, and that is not clean water. Flooded old coal workings are the source of the acid mine drainage that stains Appalachian creeks orange: low pH, iron and metals leached out of the rock. When you pump 150 million gallons of that back out of a mine, where it’s allowed to go is governed by the Clean Water Act’s NPDES permit program, and the abandoned workings themselves — unmapped hazards exactly like Saxman — are what SMCRA’s Abandoned Mine Land program exists to find, map, and seal. I’ve got no verified record of a specific pollution discharge from the rescue pumping, so I won’t invent one — but that water was a legacy environmental hazard long before it was a safety one. (H) Health — acute, it was cold. Deep mine water runs in the low 50s, and nine men stood in it and huddled for warmth for three days; hypothermia, not just drowning, was in that hole with them. All nine lived, though the foreman went up first with chest pains — and several of these men later spoke openly about the trauma they carried out of that ground. Surviving isn’t the same as walking away unharmed. Underneath all of it sits coal’s quiet killer, the black-lung and silica dust that takes miners one shift at a time whether the mine floods or not. (Tie it together) One wrong map was a safety failure, a cold-water health emergency, and a brush with a century-old environmental legacy — three faces of the same breached wall.

Mining’s present-tense killers: two dead in a single July day. On July 6, 2026, MSHA logged two mine deaths on the same date — a haul-truck driver at Freeport-McMoRan’s Morenci operation in Arizona, killed in a rear-end collision between two haul trucks, and a utility worker at U.S. Lime’s St. Clair site in Oklahoma, caught between a moving and a stationary object. Different states, different commodities, same category: mobile equipment and caught-between. So what for you: the ground gets the headlines, but the truck, the loader, and the pinch point are what actually fill the fatality reports. If you run mobile equipment, this is the week to look hard at traffic patterns, blind spots, and proximity — where do people and machines share ground, and who is watching that line.

The rule that would save miners’ lungs is still frozen. MSHA’s 2024 silica rule — the one that finally cut miners’ allowable respirable silica to 50 micrograms, half the old limit — is on hold. The Eighth Circuit stayed the compliance deadlines in April 2025, and this past April (2026) MSHA formally delayed the new requirements indefinitely, pending the court, and keeps enforcing the older, higher limits in the meantime. Miners keep breathing under the old number while advanced black lung keeps turning up in men in their 30s and 40s. So what for you: don’t wait for the limit to change to act like it already has. Dust controls, wet methods, a real respirator program, and honest sampling are legal, cheap, and available right now — the stay pauses the deadline, not the disease.

Fail of the Day

A maintenance crew needs to run new conduit through a block wall in an older plant. They do more than a lot of crews would — they pull the as-built drawings, good professional drawings, and the wall reads clear: no utilities, no lines, run your core. The tech sets the core drill and starts cutting. About halfway through, the bit bites something that shouldn’t be there — the edge of an old conduit nobody ever drew, still energized. The breaker trips. Nobody’s hurt. Every heart rate in the room doubles.

Nobody was careless here. They checked the paper and the paper said go — that’s exactly the trap. An as-built is a snapshot of what one person recorded on one day, and forty years of little changes never make it back onto the sheet. The job had a clock on it, the drawing looked authoritative, and “the print is clear” felt like permission. The fix isn’t “be more careful.” It’s making the safe way the easy way: a scan — ground-penetrating radar, a locator — before anybody penetrates a wall, floor, or slab, staged and expected so it isn’t even a decision, plus a shared understanding that a drawing is where you start looking, not the green light to cut. That’s Quecreek shrunk down to your size: they had a map too, and it was dead wrong on the other side of the wall.

Before you cut, drill, or break into anything today — a wall, a slab, a line, a space you can’t see the far side of — stop and ask one question out loud: “What’s on the other side of this, and am I trusting a drawing for the answer?” If the honest answer is a drawing somebody made years ago, that’s your cue to verify before you commit — scan it, locate it, probe it, drill ahead.
