One hundred years ago today, forty-four men went into a Pennsylvania coal mine after lunch and didn’t come out. A pocket of gas lit the fuse — but it was the fine black dust hanging in the air that turned a flash into a bomb. Today: why coal dust is the accelerant nobody sees, the rule it forced, and two things on safety leaders’ desks right now. Five minutes. Let’s go.
On This Day in Safety — August 26, 1926 · Clymer (Sample Run), Indiana County, Pennsylvania
At one o’clock in the afternoon on August 26, 1926, an explosion tore through the Clymer No. 1 mine of the CBC Coal Company, at Sample Run about a mile and a half from Clymer in Indiana County, Pennsylvania. Fifty-seven men were underground. Then, five or six minutes later — while the survivors were still trying to understand what had happened — a second explosion ripped through the workings and destroyed the First North Headings off the main entry.
When rescue crews from neighboring mines finished digging, the toll was forty-four men dead, ranging in age from seventeen to fifty-two. Nine who were working near the opening walked out uninjured. Four more were pulled out alive after being overcome underground — by the thing that kills most miners in an explosion, which isn’t the blast at all. It’s the afterdamp: the carbon monoxide and oxygen-starved air left behind, which suffocates anyone the concussion didn’t reach. Recovery of the last bodies took until that Saturday, the 29th.
The exact source of the gas was never pinned down. But investigators noted what made it so violent: the mine was dry and dusty. And that’s the lesson buried in this one, the one that took the U.S. decades and thousands of bodies to fully learn.
Name it and cite it. Here’s the chain of events that killed those men, because it’s the same chain in every coal-dust explosion: a small ignition — a pocket of methane, a spark off a tool, a shot fired wrong — makes a flash. That flash doesn’t just burn; its pressure wave lofts the fine coal dust lying on every ledge, timber, and floor of a dry mine, throwing it into the air in front of the flame. Now the flame has fuel everywhere it looks, and the “flash” becomes a self-feeding fireball that races the length of the workings. Gas starts it; dust finishes it. The defense is almost embarrassingly simple, and it’s why it took so long to mandate: rock dust. You spread pulverized limestone — inert, incombustible — over everything, so when the pressure wave lifts the dust, what goes airborne is mostly stone that can’t burn, and the chain reaction starves. Modern federal law under MSHA writes it down in numbers: 30 CFR 75.403 requires the combined coal-and-rock dust to be at least 80% incombustible, and 30 CFR 75.323 governs the methane and ventilation that keep the ignition from happening in the first place. That rule exists because of dry, dusty mines exactly like Clymer.
Translate it to the floor: this isn’t only a coal story. Anywhere fine combustible dust can pile up and get lofted — grain, sugar, flour, wood, metal, plastics — you’ve got the same physics. A little event stirs up the dust, and the dust is the real explosion. The person closest to the work is the one who sees the dust building on the beams and the ledges, and either says something or walks past it. HOP’s read on Clymer isn’t “those miners were careless.” It’s that a dry, dusty mine was a loaded gun that any small ignition could fire, and the system in 1926 had no rock-dusting mandate, no permissible-equipment standard, no real ventilation law to take the gun out of the room. How the industry answered — decades later, in blood — became the rules we work under now.
The Full EHS Picture
Most of what we cover is the “S,” but the failure is almost always Environmental, Health, and Safety at once. (E) Be honest and narrow: the 1926 blast was contained underground — there’s no documented offsite chemical release or poisoned stream from this specific explosion. But the same two things that killed these men are coal mining’s chronic environmental signature: the methane vented from coal seams is a potent greenhouse gas, and the exposed coal and pyrite that go with these operations leach sulfuric acid mine drainage into Appalachian streams for generations. (H) The health toll doesn’t end at the blast. The four men “overcome” underground were carbon-monoxide casualties — afterdamp — and the very coal dust that fueled the explosion is the same dust that scars miners’ lungs with coal workers’ pneumoconiosis, black lung, which NIOSH documents is still killing miners today, including a resurgence in severe cases among younger miners. (S) And the safety event — the explosion — was fed by that dust directly. One material, three hazards: the coal dust that detonated the mine is the dust that fills the lungs and the operation that fouls the water. Control the dust and you’re chipping at all three.

1. The CSB just closed a coke-plant explosion — and the cause was where people were standing. On August 10, 2026, the Chemical Safety Board issued its final report on the August 11, 2025 coke oven gas explosion at U.S. Steel’s Clairton Coke Works south of Pittsburgh — the largest coke plant in the Western Hemisphere. Two workers killed, eleven injured, an estimated $52.5 million in damage during a maintenance job. The findings: an ad hoc, unapproved maintenance procedure, personnel buildings sited too close to the hazard, and a process-safety management system that wasn’t doing its job. OSHA proposed $118,214 for seven serious violations. So what for safety leaders in heavy industry: two of those three causes — informal procedures and where you put your people — are things you can walk out and check today. A control that only works when everybody’s careful isn’t a control.
2. OSHA is signaling harder enforcement on repeat offenders. In its August 20, 2026 QuickTakes, OSHA — through Assistant Secretary David Keeling — paired a push for employer assistance and partnership with a blunt enforcement message: hire more compliance officers and focus enforcement on repeat violators and other bad actors. So what for safety leaders: the agency is telling you where the inspections are headed. If a hazard has already been flagged at your site and the fix didn’t stick, that’s exactly the profile enforcement is hunting. Close your open items like they’re the ones an inspector will pull first — because they are.

A woodworking shop runs a big sanding job for a rush order. The dust collector’s been throwing a breaker all week, so somebody props the bag house door and runs the line anyway to keep up. By afternoon there’s a fuzz of fine sanding dust building on the rafters, the light fixtures, the tops of the machines — the way snow settles. Nobody’s alarmed; it’s a wood shop, there’s always dust. The newest hire, sweeping under the belt sander, notices how thick it’s gotten up high and how warm the sander’s motor housing feels, and says it out loud: “if that motor sparked, all that dust up there would go.” The lead stops the job, they shut down, clean down the overhead surfaces, and don’t restart until the collector’s fixed. Nothing happened. That’s the win.
Here’s the HOP read: nobody was reckless. “There’s always dust in a wood shop” is true — context drives behavior, and everyone’s brain filed the accumulation as normal because it usually is. Propping the bag house wasn’t sabotage; it was a defense quietly failing under production pressure with nobody clocking the cost. What saved them wasn’t a rule on the wall — it was a new person close enough to the work to see the dust on the rafters as fuel instead of housekeeping, and a crew that shut down instead of pushing the order. The lesson isn’t “sweep more.” It’s that accumulated combustible dust is a pending explosion wearing a housekeeping costume.

Wherever you generate fine combustible dust — wood, grain, sugar, flour, metal, plastic, coal — walk the overhead: the beams, the ductwork, the tops of lights and cabinets, the places nobody cleans because nobody sees them. If you can write your name in the dust up there, you’ve found the fuel for the next explosion. Get it cleaned down, and fix the collector or ventilation that let it build instead of propping a door and running anyway.
