Maintenance work has a rhythm. You catch a clog, you clear the clog, you get the unit back online before anybody upstairs starts asking questions. That rhythm is exactly what killed five men at a Florida power plant — and the rulebook still carries their names.
On This Day in Safety — June 29, 2017 · Apollo Beach, Florida
Nine years ago today, five men clocked in at a Florida power plant for what should have been a maintenance job. A clog in a slag tank. Routine. By the time the sun went down, two were dead on the concrete, three more were dying in a hospital, and the furnace that cooked them had never once been shut off.
It was Tampa Electric’s Big Bend Power Station, Unit 2. A coal-fired boiler twelve stories tall. Underneath the furnace sits a water-filled slag tank — its whole job is to catch the molten slag that drips off the burning coal — glowing molten rock that runs around a thousand degrees. That day the slag had hardened and packed up the tank, top and bottom, and it wouldn’t clear.
Here’s the decision that mattered: instead of taking the unit offline to deal with it, the company sent a crew in to blast the blockage loose with high-pressure water — with the furnace still lit and running. The U.S. Department of Justice later put it flat: the written procedure for that work “could not be located,” and the men went in anyway, with no pre-job safety briefing.
Picture it from the floor. You’re standing next to a tank the size of a room, working a high-pressure lance against a wall of cooled slag, a furnace roaring overhead. You chip at the plug. The plug is the only thing holding back what’s above it. And then it lets go.
It came down like lava out of a volcano — a worker’s own words. A violent release of molten slag, thousands of gallons, gushing through the work area faster than a man can step back. Emergency crews got the call at 4:20 in the afternoon. Two men were gone where they stood. Three more died of their burns in the days and weeks after.
Their names: Michael McCort, 60, a senior plant operator who’d given Tampa Electric thirty-five years — the only company man in the group. Christopher Irvin, 40. Frank Lee Jones, 55. Antonio Navarrete, 21 — twenty-one years old. Armando Perez, 56. The last four were contractors, brought in to do the dirty work. A sixth man was burned and lived.
OSHA cited Tampa Electric for failing to follow energy-control procedures during the maintenance, and cited the contractor, Gaffin Industrial Services, for never developing procedures to control hazardous energy in the first place. Both got cited for not giving the men the burn protection that might have mattered. The Atlanta regional administrator said it plain: “This tragedy demonstrates what can happen when hazards are not properly controlled.” Years later Tampa Electric pleaded guilty to a federal crime — willfully violating an OSHA standard in a way that caused death — and a judge handed down a $500,000 fine and three years of probation.
Here’s the rule those men are written into. 29 CFR 1910.147 — the control of hazardous energy, lockout/tagout. Most people hear “lockout” and think electrical — a breaker, a padlock, a tag. But the standard is bigger than wires. Hazardous energy is anything that can move, burn, crush, or release on you: thermal, hydraulic, gravitational, chemical, stored. A tank full of thousand-degree slag held back by a crust is hazardous energy in its purest form. The rule says you isolate it, lock it, and verify it’s at zero before a single body goes in the line of fire. The companion rule, 1910.132 — personal protective equipment, says when a burn hazard is on the table, the gear to survive it has to be on the worker. Neither happened here.
And this is the part that matters: those five men were not reckless. The plug needed clearing — that was the job, and somebody had to do it. The instinct was the most normal one on any jobsite — keep the unit online, don’t eat the cost of a shutdown, clear it and move on. The system handed them that instinct and then handed them no findable procedure and no briefing to stop it. A normal pull plus a missing barrier turned a clogged tank into five funerals.
The rule is the rulebook written in those people’s names. Build the barriers — shut it down, lock it out, verify zero — so the instinct never gets the chance.

The heat program isn’t optional anymore — inspectors are out on advisory days. OSHA’s revised Heat-Related Hazards National Emphasis Program is live, and it changed the game on enforcement. On any day the National Weather Service posts a heat advisory or warning, compliance officers can run programmed inspections in targeted high-risk industries — construction, warehousing, ag, you name it. The update also added a standardized framework (Appendix J) so inspectors write heat violations the same way every time, which means fewer get tossed on a technicality. So what for safety leaders: a written heat plan in a binder won’t save you. Water that’s actually cool and close, real shade, enforced rest, and acclimatization for new and returning workers — that has to be running on the floor by Monday, because the knock can come on any hot afternoon. OSHA’s own rule of thumb for breaking in a new worker: no more than 20% of a full shift’s heat exposure on day one, stepping up about 20% a day. The first week is when they drop.
Don’t wait for the federal heat rule — it’s still stuck. The proposed national heat standard (“Heat Injury and Illness Prevention in Outdoor and Indoor Work Settings”) is still just that — proposed, and stalled in 2026 with no final rule. So what: OSHA isn’t waiting, and neither should you. They’re enforcing heat right now through the emphasis program and the General Duty Clause. The crews betting on “there’s no heat law yet” are reading it exactly backwards.
Fail of the Day
A second-day laborer on a summer pour starts slurring his words and going quiet around 2 p.m. — no stagger, no collapse, just off. A veteran on the crew clocks it, walks him to the shade trailer, gets cold water and a wet towel on his neck, and calls it in. Turns out the kid had been matching the pace of guys who’d been acclimated to that heat for months. He was a step away from heat stroke and didn’t know it, because the early signs hit your judgment before they hit your legs.
Nobody screwed up here in the way blame likes to find. The kid wanted to keep up — that’s a good instinct, not a bad one. The miss was upstream: no acclimatization plan for a new body in July, and no buddy assigned to watch him. The save came from a seasoned hand who knew what “a little off” looks like before it becomes a 911 call. That’s the whole game — build the system so the catch doesn’t depend on luck.

Before your next maintenance or cleanout task on anything that holds energy — electrical, thermal, pressure, stored, or gravity — stop and ask one question out loud with the crew: ”What’s still live, and how do we know it’s at zero?” If the answer is “we think it’s fine” or “the procedure’s around here somewhere,” you’ve found your hazard. Don’t put a hand on it until that question has a real answer.
