Five men stood around a crane boom in the Wyoming heat, and every one of them trusted the guy with thirteen years in. That trust wasn’t the problem. What killed Jeffrey Wendland was a piece of blocking that sat on a pallet a few feet away — and a system that never taught the other four why it mattered.

On This Day in Safety — July 21, 2021 · Wright, Wyoming

At 11:45 a.m. on July 21, 2021, at the Black Thunder surface coal mine in Campbell County, Wyoming, Jeffrey Wendland — a 31-year-old millwright with more than 13 years on that same mine — knelt under a 200-ton crane boom to knock out the last pin. When it let go, the steel boom structure dropped and pinned him. His crew lifted it off with a forklift, started CPR, and ran an AED. He was pronounced dead at 12:50 p.m.

The job was routine on paper: swap a 10-foot boom extension for a 30-foot one on an FMC 258 Link-Belt lattice crane. The manufacturer’s manual spelled out the sequence — lay the boom down, install two temporary pendant lines, and block the boom before you pull any pins. A warning label on both sides of the boom said it plainly: the boom can fall when the pins come out.

The pendant lines were on a pallet, a few feet away. Nobody installed them. Nobody blocked the boom. One millwright knelt between the cross beams — ten inches apart — and worked the bottom-right pin loose. He couldn’t get the left. Wendland swapped in, took the same kneeling spot, and had a coworker raise and lower the boom to free the pin. When it finally moved, gravity did the rest.

Here’s the part that matters for your crew: four of the five men on that pad had never been task-trained to change a boom extension. They deferred to Wendland because he’d done it before and he was the most experienced hand there. MSHA’s investigators were blunt about the root causes — the operator didn’t make sure the equipment was blocked against motion before people worked under it, and it didn’t task-train the crew for the job. The mine drew a 104(d)(1) citation for “unwarrantable failure.”

The rule, translated to the floor. 30 CFR 77.405(b) — “No work shall be performed under machinery or equipment that has been raised until such machinery or equipment has been securely blocked in position.” The same idea lives in OSHA’s house: 29 CFR 1926.600(a)(3) says suspended or raised equipment “shall be substantially blocked or cribbed” before anyone works under or between the parts, and 1910.147 treats gravity as exactly what it is — stored energy you have to control before you put your body in its path. Whether your card says MSHA or OSHA, the lesson is the one every one of these standards was written in blood to teach: a raised load is a loaded gun until it’s blocked. Cribbing isn’t the slow way. It’s the only way you get to stand back up.

And it wasn’t the only July 21 a machine won. One year later — July 21, 2022 — a worker at the Giant Cement plant in Harleyville, South Carolina, was killed when his right arm was caught in an auger (screw) conveyor. Different trade, different machine, same question: was the energy controlled before a human reached in?

The Full EHS Picture

Most incidents are Environmental, Health, and Safety at once — but not all, and honesty matters more than a tidy frame. (S) The safety failure here is stark and self-contained: a raised load that was never blocked, and a crew never trained to know better. (E) There was no release — nothing spilled, burned, or vented onto that dry dragline pad; this was mechanical, contained to the site. But Black Thunder is no small operation — it’s one of the largest surface coal mines in the country, and surface coal mining lives under its own environmental rulebook: reclamation under SMCRA and discharge permits under the Clean Water Act’s NPDES program, before the coal ever leaves to burn. (H) Coal’s deepest toll is the slow one. The acute hazard that day was steel; the chronic one across the industry is dust — respirable coal and silica dust that cause black lung and silicosis, diseases NIOSH has documented roaring back, including advanced cases in miners barely older than Wendland. MSHA finally cut the miner silica limit to 50 µg/m³ in 2024; legal challenges have since stalled its rollout. The same industry that took Jeffrey Wendland in a single second takes others one shift of dust at a time.

Confined space, no program, one dead worker — and a $264K bill. On July 15, 2026, OSHA cited big-rig parts distributor FleetPride for 16 serious violations after a worker asphyxiated inspecting a tanker trailer at its Corpus Christi facility. Among the findings: no confined space program at all, and a respiratory-protection program missing key elements. Proposed penalties: $264,380. So what for you: a tanker, a tank, a vault, a pit — if the atmosphere can go bad and a body can fit inside, 29 CFR 1910.146 isn’t optional paperwork. The permit is the difference between a routine inspection and a rescue that turns into a recovery. If you can’t name your confined spaces off the top of your head, that’s this week’s project.

It’s peak heat season and the NEP is live. OSHA’s updated Heat National Emphasis Program (refreshed April 10, 2026) now steers inspections toward 55 high-risk indoor and outdoor industries — and on any day the National Weather Service posts a heat advisory or warning, compliance officers can run a heat-focused inspection with no complaint required. The federal heat standard itself is still stuck in proposed-rule limbo, so don’t wait on it. Water, rest, shade, and a real acclimatization plan for new and returning workers are what a CO will look for — and what actually keeps people upright in August.

Fail of the Day

A maintenance tech jacked up a skid-mounted pump to pull a coupling, then slid underneath to reach a bolt before the cribbing was set — “just gonna be a second.” The jack drifted a quarter inch on the greasy pad and the frame settled onto a gloved hand. He walked away with a bruise, not a crush, because a second tech saw the setup, called a hard stop, and threw blocks under before anything else moved. Nobody was careless here. The setup made “hurry” the easy path: cribbing staged 40 feet away, a bolt that felt trivial, a clock running. The fix isn’t “be more careful” — it’s making the safe way the easy way. Stage the cribbing at the point of work, and make “no body under a raised load until it’s blocked” a stop-work anyone can call without a second thought — the way that second tech did.

Before your next raised-equipment job — a boom, a bed, a blade, a jacked-up frame — put your hands on the blocking first. Stage the cribbing or pins AT the point of work, not 40 feet away on a pallet, and say out loud who’s confirming it’s blocked before anyone’s body goes under.
