Good morning. It’s July — the yard’s hot and the belts are hotter. Today’s story is about the quietest killer on any job: the one you can’t see, can’t outrun, and can’t smell until it’s already made. Grab your coffee.

On This Day in Safety — July 7, 1961 · Dukla Mine, Dolní Suchá, Czechoslovakia

Sixty-five years ago today, a conveyor belt was running in a section of the Dukla mine outside Havířov where no one was working — started by accident, left alone in the dark. By the time anyone understood what it had become, 108 men underground were already lost. Not one of them was killed by the thing a miner is trained to fear.

Here’s how a normal morning turned into 108 funerals.

A miner starts a belt by accident and doesn’t notice — the kind of thing that happens a thousand times a shift. The belt runs idle. Rubber drags on stalled rollers, friction builds heat, and the heat has all morning to work. The belt catches. It’s a small fire, in a section nobody’s assigned to, so it burns unwatched. Someone catches it early — a whiff of something wrong, a suspicious smell — and sends it up the line. The report dies there. The supervisor has more urgent things in front of him and lets it go.

When the belt caught, 338 men were underground. The morning shift ends and the afternoon shift walks down past those same tunnels — into a mine that already has a fire in it — and nobody warns them, because nobody in charge has tied the smell to the flame.

Around 5 p.m., the fire is finally understood for what it is. By then it’s big, and it’s pushing smoke and carbon monoxide through the workings. Here’s the turn: the flame was never the killer. Carbon monoxide is colorless and odorless — the smell was the burning rubber, not the gas — and it doesn’t burn you. It takes oxygen’s seat in your blood until you sit down and don’t get back up. In Section 8, 108 men were cut off. The only way to starve the fire was to seal the section off from air, near midnight — and everyone who made that call knew what it meant for anyone still inside. By the time they sealed it, the chance of survival was already zero.

Seven hundred and fifty rescue workers worked that mine. The youngest man lost wasn’t yet 17. The oldest was 56. Czech mining historians who documented the disaster (Dombrovský, 2004) record the one reform it forced underground: after Dukla, the district’s mines tore out ordinary rubber belting and put in fire-resistant belt.

Name it and cite it — honestly. In the U.S., mines answer to MSHA, not OSHA. But the two lessons buried in this one don’t care what agency stamps the door. One: the whole thing started with a machine that ran when nobody meant it to. On your floor that’s 29 CFR 1910.147 — Lockout/Tagout — the standard that exists because “I didn’t know it was live” has killed more workers than malice ever has. Two: the killer wasn’t the fire, it was the air the fire made. Smoke and CO in any closed space — a tank, a vault, a pit, a tunnel — kill faster than flame. That’s the whole reason 1910.146 makes you test the air before a body goes in, and the reason a “funny smell” is never nothing.

OSHA proposes $3.52M over a Channelview cleanup — and it’s the cleanup, not the spill (OSHA, June 26). Last December, about a million gallons of sulfuric acid got loose at BWC Terminals in Channelview, Texas — some of it into the Houston Ship Channel, two people hospitalized, 44 more treated on scene. But most of the citations aren’t about the release. They’re about what came after. Inspectors say cleanup crews were sent in without hazmat training, without fit-tested respirators, and without a written emergency-response plan. One contractor alone drew $3.05 million across 18 willful-egregious violations. So what for you: the emergency doesn’t end when the leak stops. HAZWOPER (1910.120) and respiratory protection (1910.134) cover the people mopping it up, not just the people who ran the unit. If you bring contractors in to clean up, their training is your exposure — verify it before boots hit the spill, not after.

The updated Heat NEP is the reality on the ground this week (OSHA, April 10). OSHA’s revised Heat National Emphasis Program aims inspections at 55 high-risk indoor and outdoor industries, and here’s the part that bites in a July heat wave: on any day the National Weather Service posts a heat advisory or warning, a compliance officer can walk a targeted worksite at random — no complaint, no incident required. So what for you: if you’re on that list, your water-rest-shade and your new-worker plan have to be real today, not a binder on a shelf. New and returning workers are the ones dying — that’s why OSHA leans on acclimatization and the Rule of 20% (start new folks at 20% of the workload and build up over a week). The heat doesn’t care that it’s their first day. Neither will the inspector.

Fail of the Day

A millwright locks out a jammed conveyor, clears the jam, pulls his lock. Textbook. What he doesn’t know: a second feed motor upstream drives the same belt off a separate disconnect nobody had mapped. A coworker hits start from the main panel, and the belt he’d been leaning into a minute earlier lurches a foot before the E-stop grabs it. No injury. Everybody exhales.

The blameless read: the millwright did exactly what his procedure told him to. The procedure was wrong — it listed one energy source for a machine that had two. That’s not a careless worker; that’s an incomplete energy-control procedure, the kind of paperwork gap that only shows itself when a body’s already in the pinch point. 1910.147 wants every energy source identified for every machine — and the only way you find the second disconnect is to walk the line before you trust the drawing. The near-miss was the gift. The next crew might not get the foot of slack.

Pick one machine on your floor and pull its lockout procedure. Now walk the machine with the paper in your hand. Count the energy sources listed — then count the ones you can actually see and touch: disconnects, valves, air, hydraulics, stored spring or gravity. If those two numbers don’t match, you just found a Dukla in miniature — a source that can run when nobody means it to. Fix the procedure today. One machine. Before lunch.
