Eighty-six years ago today, sixty-three men went down a Pennsylvania coal mine and didn’t come back up. Most of them weren’t killed by the blast — they were killed by what came after, in the dark, while they waited for air that never arrived. Here’s what Sonman still has to teach anyone who sends people into a space where the atmosphere can turn on them.
On This Day in Safety — July 15, 1940 · Portage, Pennsylvania
At about 10:40 on a rainy Monday morning, roughly 350 men were working the Sonman “E” mine of the Sonman Shaft Coal Company outside Portage, in Cambria County. Deep in the north dip, a section of roof let go in No. 28 room — an ordinary-looking fall, except that it belched a slug of methane into the air current. The gas rode that current to where an open electric trolley locomotive was running under a bare trolley wire. The motor drew an arc. The arc found the gas.
The flame itself traveled only a small area — three entries off the north dip. Eighteen men in the immediate area walked out on their own. Twelve more on the same split of air got out unaided. But sixty-three did not. And here’s the part that should sit with every one of us: the Bureau of Mines investigators concluded that few, if any, of those men were killed outright by burns or force. About fifty-five died of afterdamp — the carbon monoxide and oxygen-starved air that fills a mine after an explosion. The flash was the event. The air was the killer.
Thirty-four of them were found together, behind a barricade of whatever they could stack, at least 700 feet in on 16 right. They’d done what miners are trained to do — wall themselves off from the bad air and wait for rescue. But the barricade was, in the report’s own words, “ineffectively erected and located,” and the recovery couldn’t reach them in time. On one of the men, searchers found a note. It showed that at least some of them were still alive at six o’clock that evening — more than seven hours after the blast. They didn’t die in a fireball. They died waiting.
Name the rule: this one runs through 30 CFR Part 75 — the federal coal-mine safety standards MSHA enforces today. Translate it to the floor. One: methane gets monitored continuously now, and the monitors cut power to the machine before a spark ever meets a pocket of gas. Two: electrical equipment that runs where gas can accumulate has to be “permissible” — built so an internal arc can’t light the atmosphere around it. An open trolley locomotive under a bare wire would never be legal at that spot today. Three: every underground coal miner carries a self-contained self-rescuer, and mines stage refuge alternatives, precisely so a man behind a barricade isn’t betting his life on afterdamp clearing before his air runs out.
Here’s the trigger — the reason those rules exist at all. In 1940, 257 coal miners died in four methane explosions; Sonman was one of them. At the time, the U.S. Bureau of Mines could investigate a disaster but had no legal right to set foot in a mine to inspect it beforehand. The bodies from that year — Bartley, Willow Grove, Sonman — are what finally pushed Congress to pass the Federal Coal Mine Inspection Act of 1941, letting federal inspectors underground for the first time. It took nearly three more decades, and a lot more funerals, to get to mandatory standards and real enforcement. Every line in Part 75 is there because someone already found out what happens without it.
The HOP read: nobody at Sonman “chose” to die. The roof fall was normal — roofs fall. The gas was normal — gassy mines make gas. The men barricading were running the right instinct with the wrong tools, because the system hadn’t handed them monitors, permissible equipment, or a rescuer on their belt. Blame the motorman and you learn nothing. Look at the conditions those men were handed and you learn everything. The people closest to the work — stacking bricks in the dark — saw the hazard clearly. They just didn’t have what they needed to survive being right.

A worker died in a crawl space — and OSHA just cited two employers for it. On July 13, OSHA announced citations against a Texas building contractor and a staffing company after a worker was killed beneath Converse Elementary School near San Antonio. He was clearing dirt out of the school’s crawl space with a mini-excavator on January 7 when he was pinned between the machine and a concrete beam. Two things in the citation should make you stop. The contractor took the rollover protective structure off the mini-excavators and bolted on fabricated parts so the machines would fit under the building — that’s the lone willful violation. And of the 15 serious violations, nearly all are confined-space failures: no evaluation of the permit-required space, no atmospheric testing, no ventilation, no rescue plan, no trained entrants. The staffing agency got cited too, for sending temp workers into that space without confined-space training. Proposed penalties: $276,399 and $23,170. So what for safety leaders — a crawl space is a confined space (29 CFR 1926 Subpart AA), and “it wouldn’t fit otherwise” is never a reason to strip a rollover cage off a machine. If you use temp labor, the host and the staffing firm share the duty to train and protect them, and OSHA will cite both.
There’s $12.7 million on the table for safety training. On July 1, the Department of Labor announced this year’s Susan Harwood Training Grants — $12.7 million for nonprofits, schools, unions, and employer groups that deliver hazard training to workers, especially in high-risk and hard-to-reach jobs. Fail of the Day
A three-person crew opens a below-grade valve vault to chase a leak. First guy pops the lid, and because it’s 8 a.m. and the last fifty vaults were fine, he swings a leg over to climb down. The newest person on the crew — three weeks in — says, “Aren’t we supposed to stick the meter in first?” Mild eye-roll. They lower the four-gas anyway. It alarms on low oxygen before the sensor even reaches the bottom of the rope. Nobody went in. Nobody got hurt. The lesson isn’t “the veteran was reckless” — he’d done it safely a hundred times, which is exactly how normal drift works. The lesson is that the newest voice on the crew caught what routine had sanded down, and the crew was built so that voice could actually stop the job. That’s the whole game.

Pick one space your people enter that everybody treats as “probably fine” — a crawl space, a valve vault, a pit, a tank, a trench box down deep. Before anyone’s body goes in today, test the air first, top to bottom, and make sure the person doing it can tell you what the readings mean and what number stops the job. Atmosphere doesn’t announce itself. Sonman’s men learned that the hard way; the family in Converse just did too. Test before you trust.
