Two men rode 300 feet up out of the earth through a hole barely wider than their own shoulders. It took fourteen days of drilling to get them there, and a third man never came up at all. Sixty-three years ago today, a caved-in anthracite mine near Sheppton, Pennsylvania taught this country how you reach a worker the ground has swallowed — a lesson still written into the rulebook you work under.
On This Day in Safety — August 13, 1963 · Sheppton, Pennsylvania
It was a small independent anthracite mine, the kind northeastern Pennsylvania was full of — a couple of men working a slope for hard coal. On August 13, 1963, David Fellin, 58, Henry “Hank” Throne, 28, and a third miner, Louis Bova, were roughly 300 feet down when the roof let go.
The collapse split them. Fellin and Throne ended up together in a pocket of the workings barely big enough to sit in. Bova was cut off somewhere on the other side of the rubble. For days nobody on the surface knew if any of the three were alive.
So they drilled. Rescuers punched a narrow six-inch borehole down toward where they thought the men might be — a needle threaded 300 feet through solid rock on the strength of a mine map and a prayer. When it broke through into open space, a voice came back up the pipe. Fellin and Throne were alive. They’d survived on water dripping off the rock. Through that six-inch lifeline crews lowered food, a hose for air, and a phone.
Then came the hard part: getting two grown men out of a hole that small. Drillers went after a rescue bore just seventeen and a half inches across — barely wider than a man’s chest — using large-diameter drilling gear brought in with the help of Howard Hughes’s people. Fourteen days after the roof came down, on August 27, they hauled Fellin and Throne up one at a time, greased to slide through, harnessed and helmeted, spinning slowly through 300 feet of black rock into daylight. Louis Bova was never found. He is entombed where he died; a Pennsylvania historical marker stands near the spot today.
Name it and cite it. Sheppton didn’t write a regulation by itself — it happened fourteen years before MSHA existed. But it sits dead center in the chain of hard lessons that built modern mine law, and it points straight at two rules that still govern the two things that nearly killed these men.
The first is the roof. In underground coal today, 30 CFR 75.200 requires an approved, written roof-control plan for the mine — engineered support, roof bolting, the whole system that keeps the top where it belongs. Sheppton’s roof came down because nothing modern was holding it up.
The second is the rescue. 30 CFR Part 49 requires mines to have trained mine-rescue teams covered and ready before an emergency — not improvised over two weeks while a family waits at the borehole. Translate it to any floor: rescue is something you plan and resource in advance, not something you invent after a worker is already down. The Federal Coal Mine Health and Safety Act of 1969 and the Mine Act of 1977 that created MSHA were written in exactly this kind of blood. And the borehole method Sheppton pioneered became the playbook — it’s how nine miners came home alive from Quecreek, Pennsylvania in 2002.
The HOP lesson is quieter, and it’s about how we respond to failure. Nobody “caused” that roof fall by being careless that morning — small anthracite operations ran on thin margins with the ground control of their day. What decided who lived wasn’t blame. It was whether anyone had a way to reach the men in time. The person closest to the work is always the first one the hazard finds. The system’s job is to already have the answer to “how do we get them out” before the question is ever asked.

(E) Environmental. This particular cave-in released nothing offsite — it was rock on rock, sealed underground. But the anthracite country that swallowed Louis Bova carries one of the longest environmental scars in America: abandoned-mine drainage. When old workings flood, water reacts with sulfur-bearing rock to form sulfuric acid that bleeds orange into the creeks — acidic enough, in places, to run near a pH of 2. By EPA and Pennsylvania DEP accounting, runoff from abandoned mines has left more than 5,500 miles of state streams impaired; a single discharge, the Old Forge Borehole near Scranton, pours 50 to 100 million gallons of that water into the Lackawanna River every day. Cleanup runs on the Clean Water Act and on Abandoned Mine Land reclamation funded under the 1977 Surface Mining Control and Reclamation Act — a bill for coal that comes due generations after the coal is gone.
(H) Health. The mine kills two ways, on two clocks. One is the roof, in seconds. The other is the dust, over thirty years — coal workers’ pneumoconiosis, black lung, which NIOSH still tracks through its Coal Workers’ Health Surveillance Program and which has been resurging, not fading. Fellin and Throne walked out; a lot of their brothers in the hard-coal region didn’t walk out of the slow disaster. Say it plainly: the same trade, the same seam, can take a worker in an afternoon or take his lungs by the time he’s sixty. Safety, environment, and health were never three departments here. They were three faces of one hole in the ground.

OSHA just doubled the clock on warehouse inspections
In its August 6 QuickTakes, OSHA announced it renewed the National Emphasis Program for warehouses and distribution centers and extended it from three years to five — with clearer authority to widen an inspection when a fatality, catastrophe, or complaint comes in. The NEP targets the usual killers on those floors: powered industrial trucks (forklifts, 1910.178), material handling and storage, walking-working surfaces and falls, blocked egress, fire protection, ergonomics, and heat. So what for safety leaders: if you run a DC or a high-volume warehouse, you’re now inside a five-year proactive-inspection window, and one complaint can legally open the door to your whole operation. Don’t wait to be selected — walk your forklift-pedestrian separation, your rack loading, and your exit routes this week like an inspector already scheduled you.
Still no federal heat rule — and it’s the middle of August
OSHA’s proposed heat standard has been sitting since the public hearing wrapped July 2, 2025 and post-hearing comments closed last October; there is still no final rule on the books in 2026. Meanwhile it’s peak-heat season and this very week (August 10–16) is OSHA’s Safe + Sound Week. So what for safety leaders: until a standard lands, heat is enforced the old way — under the General Duty Clause, Section 5(a)(1) — and OSHA is still citing employers for it. You do not need Washington to act first. Water, rest, shade, and a real acclimatization plan for new and returning workers are the difference between a hard shift and a fatality, and they’re free. Build the plan now; the heat isn’t waiting on the rulemaking.
Fail of the Day
Shared blamelessly, so the next crew doesn’t learn it the hard way.
Utility crew, laying pipe in a trench about six feet deep. The trench box was on the truck — the plan was a twenty-minute in-and-out to knock down one high spot in the floor, and the walls were “good solid clay.” Spoil pile sat right at the lip. A worker dropped in to shave the high spot, and a slab of that wall on the spoil side sloughed off without a sound — not a full burial, just enough to bury one boot to the knee and drop him. He climbed out with a wrenched ankle and a gray face. Nobody died. This time.
There’s no villain here. The box really was two minutes away, the soil really did look solid, and the job really was supposed to be quick — and every crew on earth has done the twenty-minute hop-in a thousand times without a scratch. That’s the whole trap. A trench gives no warning, and “it’ll only take a minute” is precisely the moment the box stays on the truck. The person closest to the work wasn’t reckless; he was doing the normal thing the schedule rewarded. The fix isn’t “be more careful.” It’s making the protective system non-negotiable at depth (1926.652) and keeping spoil back at least two feet from the edge (1926.651), every time — because the ground doesn’t care how short the task was.

Walk your worst hole today — the deepest trench, tank, vault, bin, or vessel your crew will enter this week — and answer three questions before anyone climbs in. One: what’s holding the walls or the roof up? Two: if that person goes down in there, exactly how do we get them out? Three: how long does that take, start to finish? Sheppton’s two survivors lived because strangers drilled through rock for fourteen days. Your crew shouldn’t need a miracle. Plan the rescue before the entry — that’s the whole point of 1926.651/.652 for excavations, 1910.146 for permit-required confined spaces, and mine rescue under 30 CFR Part 49.
