Ten years ago tonight, a gas plant on the Mississippi coast found out the hard way that a small leak isn’t always a warning shot — sometimes it’s the only notice you get before the whole thing lets go. Two workers were on shift when it blew, and the fact that they walked away owes more to luck and distance than to anything in the plan. Let’s talk about why, and what it means for your floor today.

On This Day in Safety — June 27, 2016 · Pascagoula, Mississippi

Late on the night of June 27, 2016, a brazed aluminum heat exchanger at the Enterprise Products Pascagoula Gas Plant (in Moss Point, MS) suffered a major loss of containment. Methane, ethane, and propane poured out, found an ignition source, and set off a chain of fires and explosions that shut the plant down for nearly six months. Two operators were working the night shift. By luck of where they were standing, both came home. No one off-site was hurt — though plenty of neighbors didn’t wait for an all-clear and evacuated on their own.

The CSB investigated and pinned the probable cause on thermal fatigue. A brazed aluminum heat exchanger is a block of thin aluminum passages that swap heat between cold and warm gas streams. Heat it and cool it, over and over, at different rates, and the metal expands and contracts until tiny cracks open up. Here’s the line that should stick with you: over 17 years, four of these exchangers at the plant had been repaired nine times. The operation — like much of the industry at the time — was leaning on a “leak-before-failure” assumption: the comfortable belief that a crack will always show up first as a small, fixable weep, giving you time to catch it. Pascagoula proved that bet can come up empty. The exchanger didn’t weep. It ruptured.

Name it and cite it: this is the whole reason mechanical integrity lives inside OSHA’s Process Safety Management standard — 1910.119, specifically (j). Translate it to the floor: covered equipment gets inspected and tested on a schedule built from engineering data, manufacturer guidance, and your own failure history — not on a schedule of “it hasn’t failed yet.” When a piece of equipment has been patched nine times, the repair log is trying to tell you something. The HOP read: the operator logging that same nagging leak shift after shift is the person closest to the work, and they saw it first. The only question that matters is whether the system around them treated it as a signal — a work order, an engineering review, a replacement — or as background noise everyone had learned to live with.

Heat enforcement is live — the federal rule still isn’t. OSHA’s revised Heat National Emphasis Program took effect April 10, 2026, and it’s the enforcement reality for this whole summer: a target list of 55 high-hazard industries, and compliance officers running inspections focused on heat on any day the National Weather Service posts a heat advisory or warning. The revision dropped the old numerical inspection goal — don’t read that as a stand-down. Meanwhile the permanent federal heat standard (proposed as 1910.148) is stalled; the public hearing wrapped in mid-2025 and it’s gone nowhere since. So what: stop waiting for a standard to tell you what to do. On a heat-advisory day, an officer can already expand any inspection into your heat program — or expose that you don’t have one. Water, rest, shade, acclimatization, and a written plan are the floor, today.

The CSB says get ready for the storms now. On June 2, 2026, the CSB urged chemical facilities to prepare for hurricane season before the first storm shows up — pointing to two of its own investigations: Arkema in Crosby, TX (Hurricane Harvey, 2017), where flooded refrigeration and failed backup power let organic peroxides decompose and burn, and Bio-Lab in Westlake, LA (Hurricane Laura, 2020), where rainwater reached reactive pool chemicals and pushed a chlorine plume over Interstate 10. So what: the CSB’s four priorities are your walkdown checklist — secure hazardous materials, confirm backup power for critical safety systems, train people on the emergency protocols, and coordinate with local emergency management. Do it before there’s a cone on the map, not while the wind is coming up.

Fail of the Day

A maintenance tech on a chemical transfer line keeps finding the same flange weeping a little product every few days. He wipes it, snugs it, notes it, moves on — because that’s what the last guy did, and the line “can’t” come down without blowing the day’s production targets. “Watch it” quietly becomes the plan. Then one afternoon, on a hot day with the line running warmer than usual, the gasket lets go and sprays a face-shield’s worth of product across the walkway. Nobody’s standing there. This time.

Here’s the blameless read: that tech wasn’t careless, and neither was the last guy. They were doing exactly what the system rewarded — keep it running, don’t be the one who calls the shutdown. The leak got normalized because everything around it pushed toward “live with it.” The fix isn’t “pay more attention.” It’s a hard rule with teeth: a documented leak gets a work order and a deadline, not a rag and a shrug — and calling for the shutdown is treated as a good catch, not a black mark. Change the context and you change the outcome.

Do This One Thing

Acclimatize anybody who’s “new to the heat” — and that’s more people than you think. OSHA’s own numbers: almost half of heat-related deaths happen on a worker’s first day on the job, and more than 70% happen in the first week. “New to the heat” isn’t just new hires — it’s the guy back from a week’s vacation, and it’s your whole crew during the first real heat of the season, which is right now. Run the Rule of 20%: first day in the heat, 20% of the normal workload; add 20% a day until they’re at full duration by the end of the first week. Reduce the time, not the intensity. It costs you a little production for a week. The alternative costs a lot more.
