Nineteen years ago tonight, a bridge that 141,000 people crossed every day dropped into the Mississippi River during rush hour. The crew resurfacing the deck didn’t do anything wrong. The problem was a half-inch of steel decided in 1965 — and the fact that for 40 years, nobody was required to check it.
On This Day in Safety — August 1 · Minneapolis, MN
August 1, 2007. About 6:05 p.m. The I-35W bridge — Bridge 9340, an eight-lane steel deck truss over the Mississippi in downtown Minneapolis — collapsed abutment to abutment. Roughly 1,000 feet of it came down. The center span fell more than 100 feet into the river. There were 111 vehicles on the section that dropped.
Thirteen people died. One hundred forty-five were hurt.
Twelve of the dead were drivers and passengers just going home. The thirteenth was Gregory “Jolly” Jolstad, an operating engineer for Progressive Contractors Inc. He was sitting in a skid-steer, part of an 18-person crew resurfacing the deck that evening. He’d worked for PCI for ten years and commuted 90 miles each way from Mora to do it. He was the last victim recovered — not found until August 20.
The rule it shaped. Bridges in this country get inspected under the National Bridge Inspection Standards — 23 CFR Part 650, Subpart C. Before I-35W, a routine load rating checked the big truss members. It did not require you to sit down and calculate whether the gusset plates — the connections holding those members together — could carry the load. The NTSB found that if a proper load rating had ever evaluated those connections, the half-inch error would very likely have been caught before the bridge ever opened. So the Board fired off nine recommendations to the FHWA and AASHTO, the FHWA issued a load-rating advisory for gusset plates (Technical Advisory T 5140.29), and connection evaluation got built into how bridges are rated. Translated to the floor: the joints are not an afterthought. Rate what holds it together, not just the big pieces — because that’s where it lets go.
The Full EHS Picture. Most people file I-35W under “structural failure” and stop. It was all three letters at once. (E) Environmental: more than 50 vehicles went into the Mississippi, a semi burned on the deck, and fuel, oil, and vehicle fluids leaked into a major river. EPA’s On-Scene Coordinator (Region 5) stood up a response, ran water sampling, and the Army Corps dropped the river level about two feet at the Ford Dam so divers could reach the cars — a Clean Water Act response running under the National Contingency Plan (40 CFR 300) at the same time as the body recovery. (H) Health: 145 injured, many with crush injuries, fractures, and spinal and head trauma. A school bus carrying 52 kids home from a water park went down with the span; every child lived, evacuated out the back door with help from a 20-year-old staffer named Jeremy Hernandez — and a lot of those survivors and first responders carried the psychological weight for years. The one line to hold onto: the same half-inch that killed a worker and twelve drivers also fouled the Mississippi and filled the trauma bays. One failure — E, H, and S, together. It almost always is.

The federal heat rule is still stuck in limbo — in the middle of the season it’s meant for. Per OSHA’s own rulemaking page, the proposed Heat Injury and Illness Prevention standard has now sat through its whole process — proposed in the Federal Register August 30, 2024, public comment closed January 2025, an informal public hearing that ran June 16 to July 2, 2025, and post-hearing comments that closed October 30, 2025 — and there is still no final standard. So what for safety leaders: do not wait for the rule to tell you to hand out water. OSHA is already citing heat hazards right now under the General Duty Clause, 5(a)(1), backed by its Heat National Emphasis Program. Water, rest, shade, and a real acclimatization plan for new and returning workers is the expectation today — final rule or not. It’s August. The hazard doesn’t care what stage the paperwork is at.
The CSB’s open-investigation stack keeps growing. The Chemical Safety Board currently has an open investigation into a fatal tank collapse at the Nippon Dynawave Packaging Company in Longview, Washington (incident May 26, 2026; CSB opened it May 27) — sitting alongside still-open probes of the Catalyst Refiners H2S release in West Virginia and the U.S. Steel Clairton coke-oven explosion. So what: these are the mechanical-integrity and stored-energy failures that keep killing people. When the CSB finally publishes, read the findings — the lessons show up in your plant long before they show up in a rule.
Fail of the Day
Here’s one in the spirit of tonight’s story — no names, no blame, just the lesson.
A maintenance crew needs to swap out a rooftop HVAC unit. The plan: stage the old unit, a pallet of curb adapters, and the full set of gang boxes on one bay of roof deck while everybody waits on the crane. They set it down. The deck holds. Then a foreman who’d worked that building years back walks past and says, almost offhand — “that bay’s over the old cafeteria; they cut that roof in later and never rated it for point loads.” The crew spreads the load out and moves on. Nothing happened.
That’s the whole point: nothing happened — because the person closest to the work knew something the drawings didn’t show, and said it out loud before the deck said it for him. The deck “looked strong.” So did I-35W. Looking strong is not a load rating. The win here wasn’t luck; it was a crew where a quiet heads-up was welcome instead of waved off. Do This One Thing
Before you stage a heavy load — materials, equipment, a full water tote, a lift, a loaded cart — on any deck, floor, mezzanine, scaffold, or platform, find the rated capacity and do the math against what you’re setting down. Not “it looks like it’ll hold.” The number. If you can’t find the number, that is your answer: stop and ask before you load it, not after.
