Good morning. Today we go to a tank on the Houston Ship Channel that read “safe” while it was quietly turning into a bomb — and the seventeen men who paid for an instrument nobody checked. Then two things moving right now: heat enforcement in full swing, and a federal rule that wants to take the “stop the job” button out of a worker’s hand. Let’s work.
Thirty-six years ago this week, five men clocked in for a night shift at a chemical plant and started up a compressor they’d started a hundred times before. Nothing about the job felt wrong. The one gauge that could have told them the truth had been dead for three weeks, and not one of them knew it.
On This Day in Safety — July 5, 1990 · Channelview, Texas
11:21 p.m. ARCO Chemical’s plant on the Houston Ship Channel was the biggest MTBE producer on earth. Out on the northwest edge, in a quiet utility corner, sat a 900,000-gallon wastewater tank — forty feet tall, sixty-two feet across. It held plant wastewater that carried hydrocarbons, and hydrocarbons give off vapor. Vapor plus the right amount of oxygen plus a spark is an explosion. Everybody there knew that. That’s why the tank had an oxygen analyzer reading the vapor space around the clock, and a nitrogen sweep to blow the mix inert whenever oxygen crept up.
Here’s the part that should stop you cold. Per OSHA’s investigation, that oxygen analyzer had failed on June 15 — three weeks before anybody died. With the analyzer down, the nitrogen purge backed off, and oxygen climbed inside that tank, slow and silent, day after day. The gauge on the panel kept showing numbers. The numbers were a lie.
Late June, the vapor compressor quit. Crews pulled it and went back to venting the tank to open air — the old way, before the plant added a scrubber. On July 4, they shut the nitrogen sweep off to modify piping and clean the tank. They shut it off only after the analyzer read a safe oxygen level. The analyzer was dead. It read safe because it read safe no matter what.
The next night they went to restart the compressor. Five men working right at the tank, more than usual because of the cleaning. Restarting a compressor is the most ordinary thing in a plant. That ordinary act put enough energy into a tank full of the exact wrong mixture — and it let go. The blast flattened the tank “like a soft drink can crushed in a vise,” the Los Angeles Times wrote. The 48,000-pound lid landed in a parking lot two hundred yards away. People felt it eight miles off.
Seventeen men were killed. Five ARCO Chemical employees, eleven contractors from Austin Industrial, and a vacuum-truck driver from Waste Processing found dead in his cab. Two-thirds of the dead were contract workers — the ones studies at the time found got the least safety training and knew the plant’s hazards least. Five more were hurt.
OSHA hit ARCO with $3,481,300 in fines — the largest in the agency’s history at that time — for 347 willful violations and 15 serious ones, settled January 3, 1991. Part of the deal: ARCO had to stand up a process hazard analysis team at every one of its U.S. plants. Hold onto that phrase — process hazard analysis. Channelview landed on the pile with the Phillips 66 Pasadena disaster of 1989, and that wave of process-safety catastrophes is what drove OSHA’s Process Safety Management standard — 29 CFR 1910.119 — finalized in 1992.

Mechanical integrity — 1910.119(j). The oxygen analyzer was a safety-critical instrument. It failed June 15 and stayed failed. Mechanical integrity is the rule that says the thing keeping people alive gets tested and fixed — not just watched.
Management of change — 1910.119(l). Pull the compressor, revert to atmospheric venting, shut down the nitrogen sweep. Three changes to how that tank stayed safe, and the hazard rode in on all three.
Contractor safety — 1910.119(h). Eleven of the seventeen dead worked for a contractor. The rule says their safety is the host’s business too.
Here’s the HOP read, and don’t skip it. Those men were not reckless. Somebody actually checked the gauge before shutting the nitrogen — they did the right thing. The gauge lied. A normal system and a normal, careful human instinct — trust the instrument, restart the compressor — turned one silent equipment failure into seventeen graves. The barrier was gone three weeks before the night shift ever walked in, and the system never noticed it had gone quiet.
The rule is the rulebook written in those seventeen men’s names. Build mechanical integrity so the gauge can’t lie to the next crew — verify the thing that keeps people alive, don’t just read it.

1. It’s July, the heat hammer is already out — and the “standard” is still stuck. Let’s be straight about where things stand. OSHA’s permanent Heat Injury and Illness Prevention rule is still proposed and stalled — no final date on the board. But the enforcement doesn’t wait on the rulebook. OSHA’s Heat National Emphasis Program was renewed and expanded (updated April 10, 2026) and is running now, which means on a heat-advisory day a compliance officer can open or widen an inspection and cite you under the General Duty Clause — Section 5(a)(1) — the “recognized hazard” hammer OSHA has always used on heat. So what: you don’t get to wait for a heat standard to protect people; the mechanism to hold you accountable is already live this summer. And the workers dying are overwhelmingly the new ones and the just-back-from-vacation ones. Acclimatization — OSHA and NIOSH’s Rule of 20% — is the single highest-leverage move you make this week.
2. Washington wants to take “stop the job” off the table — decide where you stand before it does. EPA has proposed rolling back the 2024 Risk Management Program rule (the “Safer Communities by Chemical Accident Prevention” rule); the proposal published February 24, 2026, and the comment window closed May 11. On the chopping block: employee stop-work authority, safer-technology-and-alternatives analysis, third-party audits, and public information sharing. So what: whatever the federal floor ends up being, the barrier that actually saves people is a worker’s confidence to shut a job down when it feels wrong — the person closest to the work sees the hazard first. That’s a culture decision, not a compliance decision. Don’t let a rulemaking tell you whether your people are allowed to stop.
Fail of the Day
Day two on a warehouse loading dock, mid-90s outside, hotter on the asphalt. New hire, twenty years old, eager — wants to prove he can hang. Nobody put him on a ramp-up; he ran the same pace as guys who’d been sweating in it for a decade. Around 2 p.m. a coworker notices he’s stopped sweating, gone quiet, fumbling the scanner. They walk him into the cooler, get water and ice on him, call it in. He came back around in twenty minutes. Twenty more and it’s a very different story.
No villain here. The kid did what eager new people always do — match the crew and say nothing. The crew did what busy crews do — assume the new guy will speak up. The heat didn’t care about anybody’s intentions. What was missing wasn’t grit; it was a plan for his body to catch up to the work. That’s the whole lesson: on his first days, his limit was lower than everyone’s around him, and no system accounted for it.
