Forty-eight years ago today, a tanker driver with twenty years behind the wheel pulled out of a refinery hauling a bomb he didn’t know he had. Nobody told him what was in the tank, that it was overloaded, or that it had no way to relieve pressure. Two hours later, 215 people were dead. Here’s what the loading rack should have caught. (A make-up issue for July 11 — we were a day behind.)

On This Day in Safety — July 11, 1978 · Alcanar, Spain

On July 11, 1978, Francisco Imbernón Villena, 50, loaded liquefied propylene at a refinery near Tarragona, Spain, and set off down the coast road. His tank was rated for 19.35 tons. It held 23.47 — more than four tons over its design limit, filled so full there was almost no vapor space left inside.

That empty space matters more than anything else on this page. Liquefied gas expands when it warms. Leave a cushion of vapor and the rising pressure stays inside the tank’s limits. Fill the tank hydraulically full, in the July sun, and the liquid has nowhere to go — so it pushes on the steel instead. By early afternoon, passing the Los Alfaques campground packed with vacationers, the overstressed tank let go.

Propylene flashed out in a white cloud, drifted into the campsite, and found an ignition source at a beachside disco. The fire raced back to the tank and the whole thing went up in a boiling-liquid expanding-vapor explosion — a BLEVE. A fireball tens of meters across. 158 people died in that first instant, the driver among them. The final toll was 215, with 200 more badly burned.

Now the part that turns a tragedy into a lesson. The inquiry found the tank had no emergency pressure-relief valves — devices once required, no longer mandatory by 1978. It found the driver had never attended the hazmat training the job called for; the company figured twenty years on the road was enough. He wasn’t told what he was carrying, wasn’t told it was overloaded, and had no gauge to check the pressure if he’d wanted to. And the refinery’s loading rack had no meter to measure what it dispensed and no automatic shutoff to stop an overfill. In the six months before the disaster, 32 tankers rolled out of that plant overloaded. This was just the one that found a campsite.

You cannot train your way out of that. The driver did nothing a careful man wouldn’t do — he drove the truck he was handed. The failure was upstream: a loading system with no meter, no shutoff, no relief valve, and no information reaching the person who’d carry the risk down the road. In the U.S., those same two hard lessons live in OSHA’s liquefied-gas rules, like 29 CFR 1910.110 — every pressure container gets a working relief valve, and you never fill it past its limit, because a liquid with no room to breathe becomes a bomb that’s only waiting on the weather. Spain learned it the expensive way and banned daytime hazmat runs through towns. The physics didn’t care that the victims were on vacation. It never does.

Heat is the hazard on the clock right now. In April, OSHA renewed and expanded its National Emphasis Program on heat (CPL 03-00-024), keeping heat inspections active through 2031 and widening the list of targeted high-risk industries. What that means on the ground this month: on days the National Weather Service posts a heat advisory or warning, an inspector can show up unannounced at a high-risk worksite and look at your heat program. So what for you: water, rest, shade, and a real acclimatization plan for new and returning workers — the ones who go down are disproportionately in their first days on the job. Don’t wait for the knock to build the plan.

Fail of the Day

A crew tops off propane forklift cylinders on a 98-degree afternoon and leaves the full bottles lined up on the sunny side of the yard, the way they always do. Mid-afternoon one starts venting — the relief valve doing exactly its job, bleeding off pressure the sun built up, hissing propane a few feet from where trucks idle. Somebody smells it, moves the bottles into the shade, and nothing burns.

Nobody did anything reckless. They filled cylinders and set them down. But “the way we always do it” put full liquefied-gas bottles in direct summer sun with no thought to expansion — and it was luck, plus a working relief valve, that turned a fire into a hiss. The fix isn’t a lecture about being careful. It’s a shaded, ventilated cylinder area and a fill practice that leaves room for the heat. Make the safe spot the closest spot, and the shortcut disappears.

Do This One Thing

Walk over to wherever you store or fill anything under pressure — propane cylinders, an LPG tank, compressed-gas bottles — and check two things: does each one have a relief valve that’s clear and pointed somewhere safe, and is anything sitting full in direct sun. Then ask whoever fills them how they know when to stop. If the answer is “you get a feel for it” instead of a gauge or a scale, you’ve found today’s fix. Four tons too full started with no way to measure.
