There’s no verified incident that lands exactly on June 26 worth your five minutes, so today’s lesson comes from this same week in safety history — June 29, 2017 — because it’s one of the most important “we knew and sent them anyway” cases on the books. Five men died clearing a clog. Here’s why.

At Tampa Electric’s (TECO) coal plant, hardened slag had jammed a tank — slag being the molten waste rock that runs off coal combustion, hot enough to reach around 1,000°F. The blockage was stuck at the top and bottom of the tank and wouldn’t clear.
The decision: bring in contractors to break the clog loose with high-pressure water blasting while the unit stayed online — live and hot. The written procedure for doing this work safely couldn’t even be located. When the blockage finally gave, molten slag surged out and poured down onto the workers below. Five men died — two at the scene, three more after being rushed to the hospital.
TECO later pleaded guilty to federal OSHA violations and was sentenced over the five deaths. Investigators and reporting laid it out plainly: the company understood the procedure was dangerous and ran it anyway.
Translation to the floor: a clog is a loaded gun. When something is plugged and there’s heat, pressure, or energy behind it, that plug is the only thing holding it back — and the moment you break it, whatever’s behind it is coming out, fast, on whoever’s in the path. You don’t stand downhill of a blockage you’re trying to free. You don’t clear it live if it can be cleared cold. And if you can’t find the written procedure for a job this dangerous, that’s not a paperwork problem — that’s a stop-work signal.
This is the spine of process safety: take the energy out before you put hands on the hazard. De-slag offline. Isolate. Verify zero energy. The hierarchy of controls says eliminate the hazard first — not hand a worker PPE and point them at it.

The line wasn’t plugged — it was pressurized behind a partial blockage. It let go and sprayed; the shield and a fast step back saved his eyes. Blameless read: he did what he’d seen done a hundred times, and nothing had gone wrong before — until the day the conditions behind the plug were different. The fix isn’t “be more careful.” It’s a rule: isolate and verify zero pressure before you crack any line, and never put your body in the line of fire of a blockage.
