Morning. Today’s story is the one the whole fire service still learns from — a routine propane transfer in a desert town that turned into the textbook BLEVE. The gas leaked without a smell. The men who ran toward it never had the doctrine that would’ve told them to run the other way. Let’s get into it.
Fifty-three years ago today, a rail car of propane sprang a hairline leak nobody could smell, and a spark of static electricity lit it. By the time the Kingman, Arizona fire department got water on the car, the clock was already running on a bomb. When the tank let go, the fireball was a thousand feet across — and eleven firefighters were gone.

It was a routine job. On the Getz siding off Route 66, a Doxol rail tank car of propane was being off-loaded into a storage tank — the kind of transfer that happened every week. What nobody knew was that a hairline crack in the side of the car was weeping gas. And here’s the part that should stop every one of us cold: the propane wasn’t odorized. There was no rotten-egg warning riding the breeze. The leak was invisible and silent until a spark of static electricity found it and lit it.
The first fire badly burned the two men working the transfer; one of them, Doxol’s own manager, would die. Now the fire fed itself. Burning propane at the valve heated the liquid still inside the car, pressure climbed, and the car’s relief valve popped — sending a second torch of flame shooting straight up. Two jets of fire, both cooking the same steel tank.
Kingman’s fire department — most of them volunteers — rolled in and did exactly what firefighters do. They put water on the car to cool it, and they hustled to set up a deluge gun that would pour on far more than their hand lines could. But in 1973, the BLEVE wasn’t in anybody’s playbook. There was no rule that said you cannot cool this fast enough — get back. That rule didn’t exist yet. It was about to be written, in the worst way there is.
Before the deluge gun was ready, the tank reached its bursting point and let go. Thousands of gallons of boiling propane flashed to vapor in an instant and ignited all at once — a fireball more than a thousand feet across, a shock wave felt more than five miles off, pieces of the rail car thrown a quarter mile. The three firefighters closest were killed where they stood. Eight more died of their burns. More than ninety onlookers gathered along the highway were hurt.
Eleven firefighters were killed. So were a gas-company man and a state trooper — thirteen lives. Among the dead: Captain William Casson, a former fire chief. Richard Williams, the principal of Kingman High School. Art Stringer, a volunteer of exactly one month. John Campbell held on for eleven days and died July 16, on the eve of a Little League tournament he was supposed to manage.
Kingman became the BLEVE the fire service learned from — photographed from a rooftop, filmed on 8mm by a vacationing family, studied in training rooms worldwide ever since. It rewrote doctrine: you do not out-brave a pressure vessel standing in flame. If you can’t flood it with cooling water, you pull everyone back to isolation distance and you let it burn. And the tank cars changed too. After the wave of propane-car BLEVEs in those years, the railroads retrofitted flammable-gas cars with thermal shields to hold the steel below failure temperature in a fire, head shields to stop punctures, and shelf couplers so cars couldn’t tear each other open. Those retrofits were finished in 1980 — and there has not been a railroad tank-car BLEVE in the United States since. On the floor, OSHA’s LP-gas standard, 29 CFR 1910.110, is the one that demands the gas be odorized so a leak announces itself, and it spells out how you store, handle, and transfer it. Kingman’s propane carried no odorant. That single barrier would have given those men their warning.
Those firefighters weren’t reckless. They did precisely what the job asks — get water on the fire, protect the town, hold the line. The system is what failed them: gas that leaked without a smell, a tank car with no thermal armor, and a fire service that didn’t have BLEVE doctrine because it was still being written that very afternoon. A normal, brave human instinct met a hazard nobody had trained for. Every rule and training film that came after is written in their names. Build the barriers — odorant, thermal protection, isolation distance, if you can’t cool it, clear it — so instinct never has to be the last line of defense.

Peak heat season is here, and the inspectors are already moving. OSHA’s revised Heat National Emphasis Program went live April 10 and runs five years. On any day the National Weather Service posts a heat advisory or the heat index clears 80°F, compliance officers can show up unannounced, walk your site, and pull your paperwork. Meanwhile the federal heat standard is still stuck in rulemaking — the proposal dropped in August 2024, the comment period closed back in October, and there’s no finish line on the calendar. Don’t let that fool you. So what for safety leaders: no standard doesn’t mean no enforcement — OSHA cites heat right now under the General Duty Clause. The written heat plan and the acclimatization schedule are what the officer asks for and what actually keeps people on their feet. Don’t wait on Washington to protect your crew.
”No penalty increase for 2026” is not OSHA going soft. For the first time in years, OSHA cancelled its annual inflation bump on fines — but only because the fall government shutdown killed the October CPI data the law requires them to use, and the statute allows no substitute. So the numbers hold right where they were: $16,550 per serious violation and $165,514 for willful or repeated. So what for safety leaders: read the freeze for what it is — a calendar quirk, not a change of heart. Fall protection was still the most-cited violation in FY25 at nearly 6,000 write-ups. The enforcement didn’t slow down. The price tag just didn’t move.
Fail of the Day
This one came without a name attached, and it doesn’t need one — because it’s played out in more shops than anybody wants to admit. A maintenance crew runs propane forklifts, and the cylinders get swapped near the back of the bay. Early in the shift, somebody catches a faint whiff of gas by the rack and says so. It gets waved off — it always smells a little back there. Normal. True, even. And that’s exactly the trap.
One newer hand couldn’t let it go. Before anyone fired up a grinder in that corner, he tracked the smell to a cylinder with a weeping fitting, shut it off, and threw the doors open. No ignition. Nobody hurt.
Notice what happened, and what didn’t. The crew that shrugged wasn’t careless — a faint smell that’s “always there” is the textbook picture of a signal gone normal. And here’s the kicker that ties straight back to Kingman: odorant can fade, soaking into new or rusty tank walls, so a weak smell can sit on top of a real leak. Blame the guy who shrugged and you learn nothing. Fix the system — gas detection at the rack, and a hard rule that any propane smell stops hot work until someone proves it’s clear — and the next crew doesn’t have to get lucky.

Next time your crew transfers a flammable — propane, fuel, solvent, anything with a flash point — bond and ground the containers before a single drop moves. Static electricity is what lit Kingman, and a bonding cable costs a few dollars against a fireball; it’s right there in the LP-gas and flammable-liquids rules, 1910.110 and 1910.106. Then burn two things into every hand on the crew: treat any gas smell as a real leak until you’ve proven otherwise, and remember that a pressure vessel sitting in fire does not get safer as the minutes pass — it gets closer to letting go. If you can’t keep it cool, you don’t need more courage. You need distance.
