Most of them were teenagers — hired for the Fourth of July rush, making good summer money. Forty-one years ago today, one stumble next to a pickup truck killed twenty-one of them before they could run. Here’s what that morning still has to teach your floor.
On This Day in Safety — June 25, 1985 · Hallett, Oklahoma
At 9:35 on a Tuesday morning, the Aerlex Fireworks plant near Jennings, Oklahoma stopped existing. A crew was unloading 20-gallon galvanized cans of finished pyrotechnic “stars” — friction-sensitive flash powder, potassium perchlorate and aluminum — sliding them across a metal truck bed into an assembly building. A worker carrying one stepped off the tailgate, his knees buckled, and he fell through the doorway. That’s all it took. The first blast leveled Building B and the truck, then rained burning stars across twenty more buildings packed with bulk explosive. The detonations were heard in Tulsa, 25 miles off. A half-mile mushroom cloud went up.
Twenty-one people died. Five survived — they were the only ones who did. Most of the dead were 16, 17, 18 years old, brought on for the holiday crush. A 16-year-old and the plant owner’s own 18-year-old son were among them.
Here’s the part that matters for your floor. The man stumbling wasn’t the cause — he was the trigger. The cause was a system that couldn’t survive a single human error. OSHA’s investigation cited 13 violations — five of them willful — and the list reads like a checklist of everything that turns one slip into a mass-casualty event: spark-producing metal containers moving friction-sensitive material, no conductive flooring or footwear, electrical gear not rated for the hazard, and — the big one — no barricades and no separation distance between buildings full of explosive. The $58,000 penalty was the largest OSHA had ever proposed against a fireworks maker.
This is the domain of 29 CFR 1910.109 — explosives and blasting agents, which governs how this material gets made, moved, and stored: bonding and grounding to bleed off static, non-sparking tools and containers, and real distance and barricades so one event can’t sympathetically set off the next. Translate it to any floor handling energetic or flammable material: people will trip, drop, and fumble — that’s normal. Your job isn’t to demand they never stumble. It’s to build the transfer so a stumble is a dropped can, not a funeral for twenty-one. After Aerlex, OSHA was ordered to inspect every fireworks manufacturer with more than ten employees. It took a leveled plant and a town full of teenagers to get there.

Washington’s worst industrial disaster in nearly a century is now a CSB case. On May 26, a roughly 900,000-gallon tank of white liquor — the hot, caustic soup that dissolves wood into pulp — failed at the Nippon Dynawave Packaging mill in Longview, releasing up to 570,000 gallons. Eleven workers were killed; the U.S. Chemical Safety Board has investigators on site. The cause is still under investigation, so nobody’s pinning it yet. So what for safety leaders: a tank that big is a mechanical-integrity question, not a “be careful” question. If you store bulk corrosive or hazardous liquid, this is the week to pull your inspection records — wall thickness, corrosion allowance, last internal — and ask whether anyone has actually laid eyes on the thing lately, or whether it’s just been quietly holding for years until it didn’t.
Heat enforcement didn’t disappear when the rule stalled. The permanent federal heat standard is parked with no finalization date, but OSHA’s revised Heat National Emphasis Program now runs through 2031, with compliance officers keyed to start asking questions around an 80°F heat index. No final rule doesn’t mean no citation — OSHA is still writing them under the General Duty Clause. So what for safety leaders: don’t wait for a standard to do the obvious. Water, shade, rest, and a real plan for acclimatizing new and returning workers will protect your crew today and keep you off the wrong end of a 5(a)(1) citation. June is when the new hire who hasn’t sweated into the job yet goes down first.

A two-person crew was topping off a steel drum with a flammable solvent off a transfer pump. The bonding clip that’s supposed to tie the drum to the line was hanging on its hook where it always hangs — because the last shift left it there, and the one before that. Mid-pour, one of them felt the little static snap on his knuckle reaching for the drum. Nothing ignited. Everybody went home.
No blame here — the clip was off because the system made “off” the path of least resistance. Nobody decided to court an explosion; they did the job the way the job had quietly drifted into being done. The near-miss is the gift: it told them what the vapor space was waiting for before it collected. They wired the clip permanently to the line so the only way to pump is bonded. That’s the fix — not “remember the clip,” but make forgetting impossible.
